[摘要]目的:探究硅酮敷料结合重组人源胶原蛋白凝胶对颌面头颈部创口瘢痕的影响。方法:回顾性分析笔者医院经重组人
源胶原蛋白凝胶治疗(对照组,49例)及硅酮敷料结合重组人源胶原蛋白凝胶治疗(观察组,49例)的行择期颌面头颈部
肿物切除术的患者临床资料。记录两组创口愈合时间、术后1个月及术后3个月瘢痕情况[温哥华瘢痕量表(Vancouver Scar
Scale,VSS)]、术后1周及术后1个月疼痛程度[视觉模拟评分(Visual Analogue Scale,VAS)]差异,并比较两组术前及术后
3个月时容貌负面评价[相貌负面身体自我量表(Negative Physical Self Scale,NPSS)]、自尊水平[Rosenberg自尊量表
(Rosenberg Self-Esteem Scale,SES)]差异。结果:观察组创口愈合时间明显低于对照组(P <0.05)。术后3个月,两组
瘢痕情况指标(各维度VSS评分)均较术后1个月降低(P <0.05),而观察组术后1个月、3个月各维度VSS评分均低于对照组
(P <0.05)。术后1个月,两组疼痛程度(VAS评分)均较术后1周降低(P <0.05),而观察组术后1周及术后1个月VAS评分
均低于对照组(P <0.05)。术后3个月,两组容貌负面评价(NPSS评分)均较术前升高(P <0.05),自尊水平(SES评分)
则较术前降低(P <0.05),且对照组变化幅度均大于观察组(P <0.05)。结论:硅酮敷料结合重组人源胶原蛋白凝胶可有
效改善颌面头颈部创口瘢痕状况,还能促进创口愈合,有效缓解患者术后疼痛,降低患者对瘢痕产生的负性情绪。

Abstract: Objective To explore the effects of silicone dressing combined with recombinant human collagen gel on wound
scars in maxillofacial head and neck. Methods The clinical data of each 49 patients who were given neoplasms resection
of maxillofacial head and neck in our hospital and treated with recombinant human collagen gel (control group) or silicone
dressing combined with recombinant human collagen gel (observation group) were retrospectively analyzed. The wound
healing time, scars condition [Vancouver Scar Scale (VSS)] at 1 month and 3 months after operation and pain degree [Visual
Analogue Scale (VAS)] at 1 week and 1 month after operation were recorded in the two groups. The negative facial evaluation
[Negative Physical Self Scale (NPSS)] and self-esteem level [Rosenberg Self-Esteem Scale (SES)] were compared between
the two groups before operation and at 3 months after operation. Results The wound healing time in observation group was
significantly lower than that in control group (P <0.05). At 3 months after operation, the scars indexes (VSS scores in each
dimension) were lower than those at 1 month after operation (P <0.05), and the VSS scores of each dimension in observation
group at 1 month and 3 months after operation were lower than those in control group (P <0.05). At 1 month after operation,
the pain degree (VAS score) in the two groups was lower than that at 1 week after operation (P <0.05), and the VAS scores
in observation group at 1 week and 1 month after operation were lower than those in control group (P <0.05). At 3 months
after operation, the negative facial evaluation (NPSS score) in the two groups was higher than that before operation (P <0.05),
and the self-esteem level (SES score) was lower than that before operation (P <0.05), and the changes in control group were
larger than those in observation group (P <0.05). Conclusion Silicone dressing combined with recombinant human collagen
gel can effectively improve the wound scars in maxillofacial head and neck, promote wound healing, and effectively relieve
postoperative pain, and it has positive significance in reducing negative emotions caused by scars.
