摘要:

[摘要]目的：分析种植固定义齿修复与种植覆盖义齿修复对无牙颌患者的满意度及美容效果。方法：回顾性分析2016年1
月-2018年1月在笔者医院进行无牙颌种植修复治疗的71例患者病例资料，根据无牙颌种植修复治疗方式的不同将71例患者分
为A组（n =33）与B组（n=38），A组患者行种植固定义齿修复，B组患者行种植覆盖义齿修复，比较两组患者种植体周围软组
织状况[改良菌斑指数（PLI）评分、改良出血指数（BI）评分、探诊深度]、主观感受（咀嚼功能、语言功能、舒适程度、
固定功能、方便程度及美观程度方面分值）及治疗满意度（治疗费用、治疗周期及治疗效果方面分值）。结果：B组种植成
功率与A组比较，无显著性差异（P ＞0.05）；B组修复后1、6、12个月种植体改良PLI评分、种植体改良BI评分与A组比较，
无显著性差异（P＞0.05）；B组修复后1、6、12个月种植体探诊深度明显高于A组(P＜0.05)；B组修复后1、6、12个月咀嚼
功能、语言功能、固定功能及舒适程度方面分值与A组比较，无显著性差异（P ＞0.05）；B组修复后1、6、12个月美观程度
方面分值明显高于A组(P ＜0.05)；B组修复后1、6、12个月方便程度方面分值明显低于A组(P ＜0.05)；B组治疗费用、治疗周
期及治疗效果方面分值明显高于A组(P ＜0.05)。结论：无牙颌种植覆盖义齿修复治疗满意度及美容效果明显优于种植固定义
齿修复，值得临床推广应用。

Abstract:

Abstract: Objective To compare the satisfaction and cosmetic effect between implant-supported overdenture and implantretained
fi xed denture for edentulous patients. Methods The clinical data of 71 edentulous patients in our hospital from
January 2016 to January 2018 were analyzed retrospectively. Patients were divided into two groups according to the
different treatment methods. Group A (n =33) received the implant-retained fixed denture, and group B (n =38) were
treated with implant-supported overdenture. Then various indexes were compared between groups, including the soft
tissue condition around the implant [plaque index (PLI) score, bleeding index (BI) score, probing depth], subjective
evaluation (mastication function, language function, comfort degree, fixation function, convenience degree,aesthetic
degree),and treatment satisfaction (treatment cost,treatment period, treatment effect). Results There was no
significant difference in implant success rate between group B and group A (P ＞0.05).There was no significant
difference in PLI score and BI score between group B and group A at 1st month, 6th month and 12th month after the
implant restoration (P ＞0.05).The probing depth in group B was signifi cantly higher than that in group A at 1st month, 6th
month and 12th month after implant restoration(P ＜0.05).The mastication function, language function, fi xation function
and comfort degree had no difference between group B and group A at 1st month, 6th month and 12th month after the
implant restoration (P ＞0.05). The aesthetic score in group B was signifi cantly higher than that in group A at 1st month, 6th
month and 12th month after implant restoration(P ＜0.05).The convenience score of group B was signifi cantly lower than that of
group A at 1st month, 6th month and 12th month after implant restoration(P ＜0.05).The treatment cost, treatment periodand
treatment effect scores of group B were signifi cantly higher than those of group A (P ＜0.05). Conclusion Compared
with implant-retained fi xed denture, application of the implant-supported overdenture has better satisfaction and cosmetic
effect amonthg edentulous patients, which is worthy of clinical application.
