摘要:

目的：探讨皮敏消胶囊联合雷公藤多苷片对湿热互结型湿疹的治疗效果及其患者血清白三烯B4（LTB4）、P物质（SP）水平变化。方法：选取2019年1月-2019年10月在笔者医院皮肤科门诊接受治疗的94例湿热互结型湿疹患者作为研究对象，采用随机数字表法将其随机分为对照组和试验组，每组47例。对照组患者采用雷公藤多苷片治疗，试验组患者在对照组基础上联合使用皮敏消胶囊治疗。比较两组的临床治疗效果，治疗前后湿疹面积及严重程度指数评分（Eczema area and severity index,EASI），生活质量评分(Dermatology life quality index,DLQI)；比较两组治疗前后血清LTB4、SP水平变化情况，记录两组患者治疗过程中发生的不良反应及复发率。结果：治疗后，试验组临床总有效率为95.74%，明显高于对照组的76.60%，差异有统计学意义（P＜0.05）；治疗前，两组患者红斑、硬肿、表皮剥落、苔藓化、皮损面积及DLQI评分比较均无显著性差异(P＞0.05)，治疗后，试验组患者各项评分改善情况均明显优于对照组，差异有统计学意义(P＜0.05)；治疗前，两组患者的血清LTB4、SP水平比较差异无统计学意义（P＞0.05）；治疗后，试验组患者血清LTB4、SP水平均明显低于对照组，差异有统计学意义(P＜0.05)。试验组患者的不良反应发生率及复发率明显低于对照组，差异有统计学意义（P＜0.05）。结论：皮敏消胶囊结合雷公藤多苷片治疗湿热互结型湿疹，更利于减轻皮损的严重程度，缩小湿疹面积，提高患者生活质量，疗效肯定，用药安全性高，且不易复发，其起效机制可能是通过下调外周血LTB4、SP减轻皮肤的炎性损害，有一定的临床应用价值。

Abstract:

Objective To investigate the clinical efficacy of Piminxiao Capsules combined with Tripterygium wilfordii in treatment of eczema and the expression level of LTB4 、SP. Methods 94 Patients with eczema from January 2019 to October 2019 were enrolled in this study.the patients were divided into control and treatment groups(n=47). The patients in the control group were administered with Tripterygium wilfordii, the patients in the treatment group were administered with Piminxiao Capsules on the basis of control group.After treatment, the efficacies were evaluated, and symptoms integral, DLQI scores and LTB4, SP in two groups were compared. Results the clinical efficacies in the control and treatment groups were76.60% and 95.74%, respectively, and there was difference between two groups (P ＜0.05), symptoms integral, DLQI scores and LTB4,SP in two groups were significantly decreased, and the differences were statistically significant in the same group (P＜0.05),and the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups(P＜0.05), the incidence of adverse reactions in the control and treatment groups were no difference between two groups（P＞0.05）, The recurernce rate within 6 months in treatment groups were significantly lower than that in the control group(P ＜0.05). Conclusion Piminxiao Capsules combined with Tripterygium wilfordii has clinical curative effect in treatment of eczema, and can reduce the levels of LTB4, SP, relieve the clinical symptoms of patients, which has a certain clinical application value.
