目的:探究股骨后踝偏心距(Posterior condylar offset,PCO)对于行高屈曲型假体全膝关节置换术(Total knee arthroplasty,TKA)患者功能恢复的影响。方法:回顾性分析2018年7月-2020年7月于笔者医院初次行单侧高屈曲型假体TKA患者73例一般资料,根据PCO值不同进行分组,PCO≤-3 mm组、-3 mm<PCO<0 mm组、0 mm≤PCO<3 mm组、PCO≥3 mm组,按顺序编为A、B、C、D组。比较各组手术前后PCO和股骨后髁偏心率(Posterior condylar offset rate,PCOR)、各个时点膝关节最大屈曲活动浓度(Range of motion,ROM)、美国膝关节协会评分(Knee society scores,KSS)和视觉模拟疼痛评分(Visual analogue score,VAS)。结果:A、B两组PCO和PCOR的术后测量值显著低于术前真实值,而C、D两组显著高于术前真实值,差异具有统计学意义(P<0.05)。术后1、3、6个月C组ROM明显大于A、B、D组,术后2周各组ROM明显小于术前和术后1、3、6个月,差异具有统计学意义(P<0.05)。术后2周各组KSS评分明显低于术前和术后1、3、6个月,术后1个月KSS评分明显高于术前,术后6个月KSS评分明显高于术后3个月,差异具有统计学意义(P<0.05)。术后各个时间点各组VAS评分明显低于术前,差异具有统计学意义(P<0.05)。结论:高屈曲型假体TKA术后PCO值增加0~3 mm,有利于提高术后ROM,但不同PCO与术后6个月内患者膝关节功能恢复和疼痛缓解无明显相关性。

Objective To explore the ankle after femoral eccentricity posterior condylar off set (PCO) for line type high buckling prosthetic total knee replacement (TKA) in patients with functional recovery. Methods Retrospective analysis was performed on the general data of 73 patients who received unilateral high-flexion prosthesis TKA for the fi rst time in our hospital from July 2018 to July 2020, and they were grouped according to different PCO values. PCO ≤-3 mm group, -3 mm<PCO<0 mm group, 0 mm<PCO<3 mm group and PCO≥3 mm group. They were named groups A, B, C, and D in that order. Observation analysis between groups before and after surgery the PCO and femoral condyle eccentricity posterior condylar off set rate (PCOR), each time Knee flexion biggest activity concentration the range of motion (ROM), the knee society score (KSS) and visual analogue score (VAS). Results The measured values of PCO and PCOR in group A and B were significantly lower than those before operation, while those in group C and D were significantly higher than those before operation. The differences were statistically significant (P <0.05). The ROM of group C was significantly higher than those of group A, B and D at 1, 3 and 6 months after operation, and the ROM of each group at 2 weeks after operation was significantly lower than that before operation and 1, 3 and 6 months after operation (P<0.05). The KSS score of each group at 2 weeks after operation was significantly lower than that before operation and at 1, 3 and 6 months after operation (P<0.05). The KSS score at 1 month after operation was significantly higher than that before operation, and the KSS score at 6 months after operation was significantly higher than that at 3 months after operation (P<0.05). The VAS score of each group at each time point after operation was significantly lower than that before operation (P<0.05). Conclusion The PCO value of high flexion prosthesis TKA increases between 0-3 mm, which is beneficial to the improvement of postoperative ROM, but there is no significant correlation between different PCO and knee joint function recovery and pain relief within 6 months after operation
