摘要:

目的：研究早期综合康复训练对断指再植患者术后再植指功能恢复及生活质量的影响。方法：选取笔者医院2018年1 月-2021年12月收治的212例断指再植患者，随机分为观察组（n=106，应用早期综合康复训练护理模式）和对照组（n=106， 应用常规护理模式）。比较两组患者再植指功能恢复情况、疼痛评分、血液循环状态、患者焦虑自评量表（Self-rating anxiety scale，SAS）、抑郁自评量表（Self-rating depression scale，SDS）、生活质量综合评定问卷（Generic quality of life inventory 74，GQOLI-74）评分及并发症发生情况。结果：护理干预后，观察组患者再植指功能恢复优良 率（99.06%）高于对照组的（93.40%）（P＜0.05）；护理干预前，两组患者再植指疼痛评分、血液循环状态、患者SAS、 SDS 、GQOLI-74评分比较差异均无统计学意义（P＞0.05）；护理干预后，两组患者疼痛评分、SAS、SDS评分均降低，且观察 组均低于对照组，差异均有统计学意义（P＜0.05）；护理干预后，两组患者再植指血液循环状态及生活质量均改善，且观 察组改善优于对照组，差异均有统计学意义（P＜0.05）；术后6个月内，观察组并发症发生率低于对照组（P＜0.05）。结 论：早期综合康复训练护理模式对于改善断指再植患者再植指功能恢复情况、降低患者疼痛程度、减少患者负性情绪、改善 患者血液循环状态、降低患者并发症发生率及提升患者生活质量方面具有积极而重要的意义，值得临床推广。

Abstract:

Objective To study the eff ects of early comprehensive rehabilitation training on the postoperative functional recovery of replanted fi ngers and quality of life in patients with severed fi nger replantation. Methods 212 patients with replantation of severed fingers admitted to the author’s hospital from January 2018 to December 2021 were randomly divided into observation group (n=106 ),using early comprehensive rehabilitation training and nursing mode and control group (n=106),using conventional nursing mode. The functional recovery of reimplanted finger,pain score,blood circulation status,Self-rating Anxiety Scale (SAS),Self-rating Depression Scale (SDS),Generic quality of life inventory 74 (GQOLI-74) scores and the incidence of complications were compared between the two groups. Results After nursing intervention,the rate of functional recovery of fi nger replantation in observation group (99.06%) was higher than that in control group (93.40%) (P ＜0.05). Before nursing intervention,there were no signifi cant diff erences in pain scores,blood circulation status,SAS,SDS and GQOLI-74 scores between the two groups (P ＞0.05). After nursing intervention,pain scores,SAS,SDS scores of 2 groups were decreased,and observation group were lower than control group,the diff erences were statistically signifi cant (P ＜0.05); After nursing intervention,the blood circulation status and quality of life of the replantation finger were improved in both groups,and the improvement in the observation group was better than that in the control group,the diff erences were statistically signifi cant (P ＜0.05). The complication rate of observation group was lower than control group 6 months after operation (P ＜0.05). Conclusion Early comprehensive rehabilitation training nursing mode has positive and important significance in improving the functional recovery of replantation fi nger (hand ),reducing the degree of pain,reducing the negative emotions of patients,improving the blood circulation status of patients,reducing the incidence of complications and improving the quality of life of patients,which is worthy of clinical promotion.
