目的:探究帐篷式骨增量技术对上前牙单牙缺失患者的治疗效果及对牙槽骨宽度的影响。方法:选取汉滨区第三人民 医院2020年2月-2021年2月收治的80例上前牙单牙缺失患者为研究对象,按治疗方法不同分为研究组(帐篷式骨增量技术) 38 例和对照组(引导骨再生技术)42例,比较两组的种植情况、红白美学指数评分、牙槽骨宽度、龈沟液骨代谢指标、牙周 情况以及术后并发症发生率。结果:种植体植入3个月后,两组患者均无种植体松动、脱落等现象,影像学检查无透影区, 种植成功率均为100%;术后6个月,研究组种植区周围红色美学指数评分高于对照组(P<0.05),两组白色美学指数评分差 异无统计学意义(P>0.05)。术后6个月,两组患者牙槽骨宽度均显著增加(P<0.05),且研究组术后6个月牙槽骨宽度高 于对照组(P<0.05);两组龈沟液中骨保护素(Osteoprotegerin,OPG)水平显著降低,骨桥蛋白(Osteopontin,OPN)水平 显著升高(P<0.05),研究组OPG水平高于对照组(P<0.05),OPN水平低于对照组(P<0.05);两组患者牙龈指数、牙菌 斑指数以及探诊深度差异均无统计学意义(P>0.05)。两组术后并发症发生率比较差异无统计学意义(P>0.05)。结论:帐 篷式骨增量技术应用于上前牙单牙缺失种植成功率较高,能提高修复后美学效果,增加牙槽骨宽度,减缓骨吸收,术后并发 症及对牙周影响均较小,值得推广应用。?/html>

Objective To investigate the therapeutic eff ect and aesthetic eff ect of tent bone augmentation technique on single upper anterior tooth loss and its influence on the width of alveolar bone. Methods 80 patients with single upper anterior tooth loss who were treated in Hanbin District Third People’s Hospital from February 2020 to February 2021 were selected and divided into study group (tent bone augmentation technique, 38 cases) and control group (guided bone regeneration, 42 cases) according to diff erent treatment methods. The implantation status, pink and white esthetic scores, alveolar bone width, gingival crevicular fl uid bone metabolism indicators, periodontal status and incidence rates of postoperative complications were compared between the two groups. Results After 3 months of implant placement, there was no implant loosening or falling off in both groups of patients. There was no transparent area in imaging examination, and the implantation success rate was 100%. At 6 months after surgery, the PES score around implantation area of study group was higher than that of control group (P <0.05), but there was no statistical signifi cance in the WES score between the two groups (P >0.05). The alveolar bone width at 6 months after surgery was signifi cantly widen in both groups (P <0.05), and the alveolar bone width of study group was wider than that of control group at 6 months after surgery (P<0.05). The OPG level in the gingival crevicular fl uid of the two groups at 6 months after surgery was signifi cantly decreased while the OPN level was signifi cantly increased (P <0.05), and the OPG in study group was higher than that in control group (P<0.05) while the OPN level was lower than that in control group (P<0.05). There were no statistical diff erences in the gingival index, plaque index and probing depth between the two groups at 6 months after surgery (P >0.05). There was no signifi cant diff erence in the incidence of postoperative complications between the two groups (P >0.05). Conclusion Tent bone augmentation technique applied to single upper anterior tooth loss has a high success rate of implantation, and can enhance the aesthetic eff ect after restoration, increase the width of alveolar bone, and slow down the bone resorption, and it has small infl uence on postoperative complications and periodontal status.
