摘要:
目的：比较正畸直丝弓矫治器联合牙合垫弹性复位固定与传统金属丝树脂夹板复位固定在外伤性牙脱位患者中的应用 价值。方法：以2019年10月-2022年7月笔者医院就诊的100例外伤性牙齿脱位患者为研究对象，采用随机数字表法将其分为 对照组（传统金属丝树脂夹板复位固定，50例）及正畸组（正畸直丝弓矫治器联合牙合垫弹性复位固定，50例）。于治疗6个 月后根据牙松动度等评价疗效，比较两组治疗前后的咬合力、龈沟液炎症因子、牙周健康及美观度。结果：正畸组的治疗总 有效率高于对照组（96.00% vs.76.00%）（P＜0.05）。治疗后，正畸组的牙松动度低于对照组（P＜0.05），正畸组的最大 咬合力、咬合速率高于对照组（P＜0.05）；治疗后，正畸组的白介素-1β、肿瘤坏死因子-α及前列腺素E2水平均低于对照 组（P＜0.05），正畸组的菌斑指数、探诊深度及出血指数均小于对照组（P＜0.05），正畸组的PES、WES评分均高于对照组 （P＜0.05）；正畸组并发症发生率低于对照组（P＜0.05）。结论：相较于传统金属丝树脂夹板复位固定，正畸直丝弓矫治 器联合牙合垫弹性复位固定治疗外伤性牙齿脱位的有效率更好，且能更好地提高咬合力，改善牙周健康，降低局部炎症水平， 并能提高牙周美观度。
Abstract:
Objective To compare the application value of orthodontic straight wire appliance combined with elastic reduction and fixation of occlusal pad and traditional metal wire resin splint reduction and fixation on patients with traumatic teeth luxation. Methods 100 patients with traumatic teeth luxation from October 2019 to July 2022 were selected as the study subjects, and they were divided into control group (50 cases, traditional metal wire resin splint reduction and fi xation) and orthodontic group (50 cases, orthodontic straight wire appliance combined with elastic reduction and fi xation of occlusal pad) by the random number table method. After 6 months of treatment, the therapeutic effi cacy was evaluated according to tooth mobility, and the occlusal force, gingival crevicular fl uid infl ammatory factors, periodontal health and aesthetics were compared between both groups before and after treatment. Results The total eff ective rate of treatment in orthodontic group was higher than that in control group (96.00% vs. 76.00%) (P ＜0.05). The tooth mobility was lower in orthodontic group than that in control group (P＜0.05). The maximum occlusal force and occlusal rate in orthodontic group were higher compared with those in control group (P＜0.05). The levels of interleukin-1β, tumor necrosis factor-α and prostaglandin E2 in orthodontic group after treatment were lower than those in control group (P＜0.05). After treatment, the plaque index, probing depth and bleeding index were lower in orthodontic group compared to control group (P＜0.05). PES score and WES score in orthodontic group were higher than those in control group (P＜0.05). The incidence rates of complications were lower in orthodontic group than those in control group (P＜0.05). Conclusion Compared with traditional metal wire resin splint reduction and fi xation, orthodontic straight wire appliance combined with elastic reduction and fixation of occlusal pad is more effective in the treatment of traumatic teeth luxation, and the latter one can better enhance the occlusal force, improve the periodontal health, reduce the local infl ammation level, and promote the periodontal aesthetics.
