摘要:
目的:分析比较数字化印模与传统印模用于单颗后牙种植修复美学效果及对咀嚼功能的影响。方法:选取2021年11 月-2022年7月笔者医院收治的80例(患牙80颗)需要单颗后磨牙种植的患者为研究对象,按照单双号法随机分为数字化组 (单号,40例,40颗患牙)与传统组(双号,40例,40颗患牙),数字化组采用数字化印模方式取模修复,传统组采用传 统印模方式取模修复,比较两组临床操作时间、术中不适感;两组种植修复后即刻、种植修复后第6个月、12个月的修复效 果(外观、颜色改变以及边缘贴合性);比较两组种植前、种植修复后3个月的咀嚼功能(咀嚼效率与咬合力)、牙周 健康指标[菌斑指数(Plaque index,PLI)、牙周袋深度、出血指数(Bleeding index,BI)]及龈沟液炎症指标[肿 瘤坏死因子(Tumor necrosis factor,TNF-α)与高敏-C反应蛋白(High-sensitivity C-reactive protein,hsCRP)],对比两组种植修复后12个月的美观度[美学量表(Aesthetic component,AC)评估]。结果:数字化组临床操 作时间与术中疼痛评分均显著低于传统组(P<0.05);种植修复后即刻,两组外观、颜色改变及边缘贴合性比较差异无 统计学意义(P>0.05);种植修复后第6个月、第12个月两组外观、颜色改变比较差异无统计学意义(P>0.05);种植 修复后第6个月、第12个月数字化组边缘贴合性显著优于传统组(P<0.05);种植修复后第3个月,两组咀嚼效率与咬合 力明显升高(P<0.05),且数字化组显著大于传统组(P<0.05);种植修复后第3个月,两组PLI、BI、牙周袋深度均 显著下降(P<0.05),且数字化组显著低于传统组(P<0.05);种植修复后第3个月,两组TNF-α、hs-CRP水平均明显 降低(P<0.05),且数字化组显著小于传统组(P<0.05);种植修复后第12个月,数字化组AC评分中牙齿形态、颜色、 美观度及总评分均显著高于传统组(P<0.05)。结论:相较于传统印模,单颗后磨牙种植修复中采用数字化印模精确性更 高,改善患牙咀嚼功能、牙周健康及龈沟液炎症反应更好,远期修复效果及种植体美观度更高,值得推荐。?/html>
Abstract:
Objective To explore the aesthetic eff ect of digital impression and traditional impression on single posterior implant restoration and its effect on chewing and occlusal function. Methods 80 patients (80 affected teeth) who needed a single posterior molar implant from November 2022 to July 2022,were randomly divided into digital group (single number, n=40, 40 aff ected teeth) and traditional group (double number, n=40, 40 aff ected teeth) according to the single and double number method. The digital group was repaired by digital impression method, and the traditional group was repaired by traditional impression method. The clinical operation time and intraoperative discomfort were compared between the two groups. The repair eff ect (appearance, color change and edge adhesion) of the two groups immediately after implantation, 6 months and 12 months after implantation, the chewing function (chewing effi ciency and bite force), periodontal health indicators [plaque index (PLI), periodontal pocket depth, bleeding index (BI)] and gingival crevicular fluid inflammation indicators [tumor necrosis factor (TNF-α) and high-sensitivity-C-reactive protein ( hs-CRP)] and aesthetics [aesthetic scale (AC) evaluation] at 12 months after implantation were compared between the two groups. Results The clinical operation time and intraoperative pain score of the digital group were signifi cantly lower than those of the traditional group (P <0.05). Immediately after implantation, there were no signifi cant diff erences in appearance, color change and edge fi t between the two groups (P >0.05). At 6 months and 12 months after implantation, there were no signifi cant diff erences in appearance and color change between the two groups (P >0.05). The edge fi t of the numerical group was signifi cantly better than that of the traditional group at 6 and 12 months after implantation (P <0.05). After 3 months of implantation, the masticatory effi ciency and bite force of the two groups increased signifi cantly (P <0.05), and the digital group was signifi cantly higher than the traditional group (P <0.05). At 3 months after implantation, the PLI, BI and periodontal pocket depth of the two groups decreased signifi cantly (P <0.05), and the digital group was signifi cantly lower than the traditional group (P <0.05). At 3 months after implantation, the levels of TNF-α and hsCRP in the two groups were signifi cantly decreased (P <0.05), and the levels in the digital group were signifi cantly lower than those in the traditional group (P <0.05). At the 12 th month after implantation, the tooth shape, color, aesthetics and total score of AC score in the digital group were signifi cantly higher than those in the traditional group (P <0.05). Conclusion Compared with traditional impression, digital impression used in the single posterior molar implant restoration has higher accuracy, better improvement of the masticatory function, periodontal health and gingival crevicular fl uid infl ammation, and higher long-term repair eff ect and implant aesthetics, which is worthy of recommendation.
