摘要:

目的：采用Sommerlad-Furlow法对一期腭裂患者进行腭裂整复术，并对其手术方式及效果进行探讨分析。方法：共收集一期腭裂患者124例，其中双侧/单侧完全性腭裂（B/UCCP）24例，硬软腭裂（HSCP）77例，软腭裂/腭隐裂（SCP/SMCP）23例，手术方法是采用鼻咽旁松弛切口减轻鼻腔黏膜层的张力，以Sommerlad法进行腭帆提肌的解剖复位和重建，运用Furlow法进行有效的软腭延长，即Sommerlad-Furlow法。手术由3位医师（S1/S2/S3）完成，临床工作时间分别为3年、5年及10年。统计手术时长、软腭延长长度、松弛切口率、腭瘘率，采用SPSS 21.0软件包进行统计学分析。结果：手术时长平均为（74.21±28.376）min，其中S3手术时长明显短于S1、S2（P＜0.05）；软腭延长长度平均为（10.31±1.417）mm，其中S1延长长度短于S2、S3（P＜0.05），而S2、S3间差异无统计学意义（P＞0.05）；总体行松弛切口率为4.84%，均为B/UCCP患者，S1、S2、S3之间差异无统计学意义（P＞0.05）；总体腭瘘率为7.26%，其中B/UCCP患者7例，HSCP患者2例，S1、S2、S3之间差异无统计学意义（P＞0.05）。结论：不同临床经验的医师，均可掌握Sommerlad-Furlow法手术操作技巧，在术中有效延长软腭长度，降低松弛切口率，减少腭瘘率，为提高腭咽闭合率打下良好基础。

Abstract:

Objective To use the Sommerlad-Furlow method for cleft palate repair in patients with primary cleft palate, and to explore and analyze the surgical methods and effects. Methods A total of 124 patients with primary cleft palate were collected, including 24 cases of bilateral/unilateral complete cleft palate (B/UCCP), 77 cases of hard and soft cleft palate (HSCP), 23 cases of soft cleft palate/cleft palate (SCP/SMCP). The surgical method was to reduce the tension of nasal mucosa by paranasopharyngeal relaxation incision. The levator veli palatini muscle was anatomized and reconstructed by Sommerlad method. The eff ective soft palate extension was performed by Furlow method, that is Sommerlad-Furlow method. The surgery was completed by three physicians (S1/S2/S3), with clinical working hours of 3, 5, and 10 years, respectively. Statistical analysis was conducted using SPSS 21.0 software package to calculate the duration of surgery, the length of soft palate extension, the rate of relaxed incisions, and the rate of palatal fistula. Results The average surgical duration was (74.21±28.376) minutes, with S3 signifi cantly shorter than S1 and S2 (P＜0.05). The average extension length of the soft palate was (10.31±1.417) mm, with S1 being shorter than S2 and S3 (P＜0.05), while there was no statistical diff erence between S2 and S3 (P＞0.05). The overall rate of relaxed incision was 4.84%, all of which were B/UCCP patients. There was no statistical diff erence between S1, S2, and S3 (P＞0.05). The overall rate of palatal fi stula was 7.26%, with 7 cases of B/UCCP and 2 cases of HSCP. There was no statistical diff erence between S1, S2, and S3 (P＞0.05). Conclusion Physicians with diff erent clinical experiences can master the surgical techniques of the Sommerlad-Furlow method, eff ectively extending the length of the soft palate, reducing the rate of loose incisions, and reducing the rate of palatal fi stula during surgery, laying a good foundation for improving the velopharyngeal closure rate
