摘要:

目的：探讨固定-活动义齿修复中青年牙周病伴牙列缺损患者的临床效果。方法：选择2018年10月-2022年2月在西宁 市第一人民医院口腔外科治疗的160例中青年牙周病伴牙列缺损患者为研究对象，采用随机数字表法分为观察组和对照组， 每组80例。对照组使用活动义齿修复治疗，观察组使用固定-活动义齿联合修复治疗，比较两组患者的修复效果、美学效 果[根面覆盖美学评分（Root coverage esthetic score，RES）、黏膜瘢痕指数（Mucosal scarring index，MSI）]、 口腔健康状态[口腔健康评估表（Oral health assessment tool，OHAT）]、牙周健康状况、口腔健康管理[口腔保健自 我效能量表（Self-efficacy scale for self-care，SESS）]及咀嚼功能。结果：治疗后，观察组患者修复效果好于对照 组（P＜0.05）；两组患者RES评分均升高，且观察组高于对照组（P＜0.05），两组患者MSI评分、OHAT均降低，且观察组低 于对照组（P＜0.05）。治疗后，两组患者牙周健康状况相关指标评分均降低，且观察组低于对照组（P＜0.05）；观察组患 者SESS评分优于对照组（P＜0.05）；两组患者咀嚼功能评分均升高，且观察组高于对照组（P＜0.05）。结论：采用固定- 活动义齿修复中青年牙周病伴牙列缺损患者，可提高修复及美学效果，改善患者口腔健康状态，提升其生活质量。

Abstract:

Objective To study the clinical eff ects of fi xed-removable prostheses in the treatment of periodontal disease with dentition defect in young and middle-aged adults. Methods 160 young and middle-aged patients with periodontal disease complicated with dentition defect who were treated in the hospital from October 2018 to February 2022 were selected as the study subjects, and were divided into observation group and control group by the random number table method, with 80 cases in each group. The control group was treated with removable denture repair, and the observation group was given fi xed denture combined with removable denture. The repair effect, aesthetic effect [root coverage esthetic score (RES), mucosal scarring index (MSI)], oral health status [Oral health assessment tool (OHAT)], periodontal health status, oral health management [Selfeffi cacy scale for self-care (SESS)] and masticatory function were compared between the two groups. Results After treatment, the repair effect in observation group was better than that in control group (P＜0.05); RES score in the two groups was increased, and the score in observation group was higher than that in control group (P＜0.05), and the MSI score in both groups was decreased, and the score in observation group was lower than that in control group (P＜0.05). The OHAT score in both groups was decreased after treatment, and the score in observation group was lower than that in control group (P＜0.05). The scores of periodontal health status-related indexes in the two groups were declined, and the scores were lower in observation group than those in control group (P＜0.05). After treatment, SESS score in observation group was better than that in control group (P＜0.05). The masticatory function score in both groups was increased, and the score in observation group was higher than that in control group (P＜0.05). Conclusion Fixed-removable prostheses in the treatment of periodontal disease with dentition defect in young and middle-aged patients can improve the repair eff ect, enhance the aesthetic eff ect, promote the oral health status and enhance the quality of life of patients.
