[摘要]目的：探讨双重固定联合眼轮匝肌后脂肪部分切除在重睑成形术中的应用。方法：选取2017年1月-2018年6月笔者医
院收治的65例存在上睑肥厚、臃肿等单睑就医者，实施重睑成形手术。在手术切口的上唇切除部分眼轮匝肌后，处于眶隔前
以及眶上缘外侧骨膜前的脂肪组织，适当对部分肥厚的眶隔脂肪进行切除；在手术切口的下唇对一部分肥厚的睑板前纤维
脂肪组织进行切除。采用双重固定，即第一重固定是将下唇的真皮-眼轮匝肌固定于睑板或上睑提肌腱膜，而第二重固定是
采用皮肤-眶隔膜-皮肤的方式进行。术后随访观察其疗效。结果：65例就医者上睑臃肿的情况得到良好恢复与改善，2例术
后重睑皱褶变浅，必须再次手术进行修复与改善；1例术后出现上睑局部小血肿，10d后自行缓解消肿；3例术后眉外侧上方
部分区域出现麻木的情况，3个月后自行恢复与缓解。其他就医者均恢复良好，手术切口痕迹不明显，睁闭眼时上睑合开自
然。结论：采用双重固定联合眼轮匝肌后脂肪部分切除，对上睑肥厚、臃肿单睑者行重睑成形手术，可有效改善上睑臃肿情
况，同时形成安全稳定且长久的动态重睑。

Abstract:
Abstract: Objective To investigate the application of double fixation combined with partial fat resection of orbicularis oculi
in double eyelid plasty. Methods 65 patients with hypertrophy and bloated upper eyelid were selected to perform double
blepharoplasty from January 2017 to June 2018. The fat tissue in the anterior orbital septum and the lateral periosteum of the
supraorbital margin after the resection of part of the orbicularis oculi muscle through the upper lip of the surgical incision was
properly resected to the partial hypertrophic orbital septum fat. A portion of hypertrophic pretarsal fibrous adipose tissue was
removed from the lower lip of the surgical incision. The double fixation method is used to form a safe, stable and long-lasting
dynamic double eyelid fold, that is, the first fixation is to fix the dermis orbicularis oculi muscle of the lower lip to the tarsal
plate or the upper eyelid levator tendon membrane. The second fixation was done by skin-orbital septum-skin. The followup
period was 3 months to 2 years. Results The condition of bloated upper eyelid in all the patients was well recovered and
improved. The double eyelid fold became shallow in 2 cases after operation, which must be repaired and improved again. In
one case, the local small hematoma of the upper eyelid appeared after the operation, and the swelling was relieved after 10d.
There were 3 cases of numbness in the part of the lateral eyebrow above the operation, the patients recovered and remitted by
themselves after 3 months. The other patients recovered well, the marks of surgical incision were not obvious, sand the upper
eyelid was opened naturally when the eyes were open or closed. Conclusion Double fixation combined with partial excision
of posterior fat of orbicularis oculi muscle was used to perform double blepharoplasty for those with hypertrophy and bloated
upper eyelid to improve the condition of bloated upper eyelids and to form stable and stable dynamic double eyelids.
