[摘要]目的:探讨引导骨再生术(GBR)对不同牙龈生物型种植红白美学效果的影响。方法:选取2015年1月-2017年1月笔者
医院收治的美学区行延期种植修复并同期行GBR的180例患者,采用同名牙探诊法进行牙龈生物型分类,随机分为A组(厚龈
型)和B组(薄龈型),其中A组96例,B组84例,分别对两组种植体植入术前、植入术后基台戴入前牙槽嵴顶、牙槽嵴顶下
3mm牙龈厚度进行测量;于烤瓷全冠戴入完成后第0、6、9个月,分别对两组美学区进行红色美学指数(PES)、牙冠白色美
学指数(WES)进行评分。结果:A组术前、术后种植位点牙槽嵴顶下3mm牙龈厚度(2.18±0.27)mm>1.5mm,差异有统计学
意义(P <0.05);B组术前种植位点牙槽嵴顶下3mm牙龈厚度(1.36±0.37)mm<1.5mm,B组术后种植位点牙槽嵴顶下3mm牙
龈厚度(1.82±0.17)mm>1.5mm,差异有统计学意义(P <0.05);A、B两组术前、术后种植位点牙槽嵴顶处牙龈厚度均>
1.5mm,差异有统计学意义(P <0.05);A组PES评分显著优于B组,差异有统计学意义(P <0.05);PES评分与WES评分存在
相关性;A、B两组组内0、6、9个月PES评分、WES评分相比,差异均有统计学意义(P <0.05);种植术后牙槽嵴顶牙龈厚度
与PES+WES评分有相关性。结论:同期行GBR术的延长种植修复能显著增加种植区牙槽嵴顶及牙槽嵴顶下3mm的牙龈厚度,厚
龈型生物型患者在同期行GBR术延期种植的红色美学效果优于薄龈型生物型患者,不同牙龈生物型在同期行GBR术延期种植的
红色美学指数与白色美学指数呈正相关。
Abstract:
 
Abstract: Objective To explore the effect of guided bone regeneration (GBR) on the aesthetic effect of red and white in
different gingival biotypes. Methods A total of 180 patients with GBR were enrolled in the aesthetic district admitted to our
hospital from January 2015 to January 2017. The gingival biotypes were classified by the same name dental exploration method
and randomly divided into group A (thickness type), group B (thin sputum type), including 96 cases in group A and 84 cases in
group B. Before the implantation of implants and implants, the anterior alveolar dome and alveolar dome were inserted into the
abutment. The thickness of the lower 3 mm gingiva was measured; the red aesthetic index (PES) and the white aesthetic index (WES)
of the two groups were scored in the 0, 6 and 9 months after the completion of the full crown of the porcelain. Results In group
A, the thickness of 3mm gingiva under the alveolar crest was (2.18±0.27) mm>1.5mm, the difference was statistically significant
(P <0.05). The preoperative implantation site of the group B was under the alveolar crest. The thickness of 3mm gingiva was
(1.36±0.37) mm<1.5mm. The thickness of 3mm gingiva under the alveolar crest of group B was (1.82±0.17) mm>1.5mm, the
difference was statistically significant (P <0.05). The thickness of the gingiva at the alveolar crest of the two groups before and
after operation was>1.5 mm, the difference was statistically significant (P <0.05). The PES score of group A was significantly
better than that of group B, the difference was statistically significant ( P <0.05); PES score was correlated with WES score;
PES score and WES score were statistically significant at 0, 6 and 9 months in group A and B (P <0.05); The thickness of the
posterior alveolar crest gingival was correlated with the PES+WES score. Conclusion The extended implant restoration of
GBR in the same period can significantly increase the thickness of the gingiva of the alveolar crest and the alveolar crest under
the planting area. The red aesthetic effect of the patients with thick-flat biotypes during the same period of GBR is better than
that of the thin-type organism. In patients with type, the red aesthetic index of different periodontal biotypes in the delayed GBR
was positively correlated with the white aesthetic index.
