[摘要]目的:探讨不同拔牙模式对安氏Ⅰ类错牙合患者软硬组织A、B点的影响。方法:选取安氏Ⅰ类错牙合患者164例,采
用随机数字表法分为实验组与对照组,每组82例。实验组患者拔除4颗第一前磨牙,对照组患者拔除4颗第二前磨牙,拔
除后两组患者均不使用任何辅助矫正器或支抗装置。比较两组患者矫治前后软硬组织A、B点指标及软组织变化。结果:
与矫治前比较,矫治后两组患者A’-Y、B’-Y、UL-Y及LL-Y测量值均明显减少(P <0.01),且实验组明显小于对照组
(P <0.01)。与矫治前比较,矫治后对照组患者上下唇倾角及实验组患者鼻唇角、上下唇倾角、上唇翻卷度测量值均明显
增加(P<0.01),且实验组鼻唇角、颏唇角、下唇倾角及上下唇翻卷度测量值明显大于对照组(P <0.01)。结论:不同拔
牙模式对安氏Ⅰ类错牙合患者上下唇凹点及上下唇突点相对X/Y轴移动距离的影响不同,拔除第一前磨牙对患者上下唇凹点/突
点相对Y轴的距离及鼻唇角、下唇倾角、上下唇翻卷度的影响大于拔除第二前磨牙。
Abstract:
Abstract: Objective To investigate the influence of different extraction modes on A and B points of soft and hard tissue in
angle class I malocclusion patients. Methods 164 cases of angle class I malocclusion patients were selected and divided into
the experiment group and the control group according to the random number table methods, 82 cases in each group. 4 first
premolars were removed in the experiment group, 4 second premolars were removed in the control group, after the removal,
the 2 groups of patients did not use any auxiliary aligners or anchorage devices. The changes of A and B points of soft and
hard tissue and the soft tissue changes before and after orthodontic treatment were compared between the 2 groups. Results
Compared with before orthodontic treatment, the measured values of A’-Y, B’-Y, UL-Y and LL-Y in the 2 groups significantly
decreased after orthodontic treatment (P <0.01), and the experiment group was significantly smaller than that of the control
group (P <0.01). Compared with before orthodontic treatment, the measured values of upper and lower lip inclination angle in
the control group and nasolabial angle, upper and lower lip inclination angle, upper lip rollover degree in the experiment group
significantly increased after orthodontic treatment (P <0.01), and the measured values of nasolabial angle, labrumlip angle,
lower lip inclination angle, upper and lower lip rollover degree in the experiment group were significantly greater than that of
the control group (P <0.01). Conclusion Different extraction modes have different effects on the movement distances of the
upper and lower lip pits and protrusion points relative to the X/Y axis in angle class I malocclusion patients, the influence of
removal of the first premolar to the distances of the upper and lower lip pits and protrusion points relative to the Y axis and the
nasolabial angle, lower lip inclination angle, upper and lower lips turnover is greater than removal of the second premolar.
