[摘要]目的:探究二氧化碳(CO2)激光联合不同剂量干扰素治疗尖锐湿疣(CA)的临床疗效。方法:回顾性分析82例行CO2
激光治疗的CA患者临床资料,根据不同干扰素封闭注射给药剂量分为大剂量组(3×106IU/次,n =28)、小剂量组(1×106
IU/次,n =31)与对照组(未给予干扰素,n =23)。比较激光治疗前及治疗后4周,三组外周血免疫细胞分布[CD3+、CD4+/
CD8+、自然杀伤(NK)细胞]、白细胞介素[白细胞介素-2(IL-2)、白细胞介素-10(IL-10)、白细胞介素-12(IL-
12)]、功能性细胞因子[淋巴细胞趋化因子(LTN)、巨噬细胞移动抑制因子(MIF)、粒细胞-巨噬细胞集落刺激因子(GMCSF)]
水平变化,分析三组激光治疗后4周时治疗效果、激光治疗后3个月内复发率,及两组用药患者给药疗程内的药物不良
反应发生情况。结果:激光治疗后4周,三组外周血CD3+水平均较激光治疗前显著提升(P <0.05),但组间同一时间CD3+水
平比较均无统计学意义(P >0.05);三组外周血CD4+/CD8+、NK细胞及血清IL-2、IL-12、LTN水平均较激光治疗前显著
提升,且同一时间比较大剂量组>小剂量组>对照组(P <0.05);三组血清IL-10、MIF、GM-CSF水平均较激光治疗前
显著下降,其同一时间比较大剂量组<小剂量组<对照组(P <0.05);大剂量组与小剂量组治疗有效率均明显高于对照
组(P <0.05)。激光治疗后3个月内,大剂量组与小剂量组复发率均明显低于对照组(P<0.05)。给药疗程内,大剂量组
药物不良反应总发生率明显高于小剂量组(P <0.05)。结论:CO2激光联合干扰素封闭注射治疗CA能取得较为理想的临床效
果,且疗效与复发率不受给药剂量影响,采取小剂量给药方案安全性更突出,有利于患者预后及康复。
Abstract:
Abstract: Objective To investigate the clinical effects of carbon dioxide (CO2) laser combined with different doses of interferon
in the treatment of condyloma acuminatum (CA). Methods The clinical data of 82 patients with CA who underwent CO2 laser
treatment were retrospectively analyzed. According to the different administration doses of interferon-blocked injection, the
patients were divided into high-dose group (3×106 IU/time, n =28) and low-dose group (1×106 IU/ time, n =31) and control group
(not given interferon, n =23). The levels of peripheral blood immune cell distribution [CD3+, CD4+/CD8+, natural killer (NK)
cells], interleukins [interleukin-2 (IL-2), interleukin-10 (IL-10), interleukin-12 (IL-12)] and functional cytokines [lymphocyte
chemotactic factor (LTN), macrophage migration inhibitory factor (MIF), granulocyte-macrophagocyte colony-stimulating
factor (GM-CSF)] were compared among the three groups before laser treatment and at 4 weeks after laser treatment. The
treatment effects at 4 weeks after laser treatment, the recurrence rate within 3 months after laser treatment, and the occurrence
of adverse drug reactions during the course of treatment were analyzed among the three groups. Results At 4 weeks after
laser treatment, the CD3+ level in peripheral blood in the three groups was significantly higher than that before laser treatment
(P <0.05), but there was no significant difference in CD3+ level between the groups at the same time (P >0.05). The levels of
peripheral blood CD4+/CD8+, NK cells and serum IL-2, IL-12 and LTN in the three groups were significantly higher than those
before laser treatment, and the levels at the same time showed high-dose group>low-dose group>control group (P <0.05).
The levels of serum IL-10, MIF and GM-CSF in the three groups were significantly lower than those before laser treatment,
and the levels at the same time showed high-dose group<low-dose group<control group (P <0.05). The effective rate of
treatment in high-dose group and low-dose group was significantly higher than that in control group (P <0.05). Within 3
months after laser treatment, the recurrence rate in high-dose group and low-dose group was significantly lower than that in
control group (P <0.05). Within course, the total incidence rate of adverse drug reactions in high-dose group was significantly
higher than that in low-dose group (P <0.05). Conclusion CO2 laser combined with interferon-blocked injection for CA can
achieve ideal clinical effects, and its efficacy and recurrence rate are not affected by the doses. And low-dose regimen has more
prominent safety, and it is beneficial to the prognosis and rehabilitation.
