[摘要]目的:建立下颌骨颏部-体部层片状骨折坚固内固定的三维有限元模型,分析功能载荷下下颌骨的生物力学变化规
律。方法:基于健康志愿者的下颌骨CT数据建立下颌骨三维有限元模型,在此模型上模拟出下颌骨颏部-体部(右侧)层片
状骨折,并模拟小型钛板坚固内固定。依据内固定螺钉的类型分为单皮质钉固定和双皮质钉固定模式,依据载荷的类型分为
右侧咀嚼载荷、左侧咀嚼载荷及双侧咀嚼载荷,共计6种工况。分析每种工况下骨折断端间的位移情况及内固定接骨板的Von
Mises应力分布情况。结果:双皮质钉固定时骨断端间相对位移小于单皮质钉固定,患侧咀嚼时骨断端间的相对位移最大,其
中单皮质钉固定右侧咀嚼时骨折断端间的相对位移达到了0.158mm,超过了临床内固定要求的警戒线0.150mm。双皮质钉固定
时内固定接骨板的应力集中程度较单皮质钉固定轻;双侧咀嚼较单侧咀嚼的应力集中程度轻。结论:对于下颌骨颏部-体部
层片状骨折,采用双皮质钉的坚固内固定方式较单皮质钉固定方式力学稳定性好,更有利于为骨折愈合创造良好的生物力学
环境。

Abstract:
Abstract: Objective To establish a three-dimensional finite element model for rigid internal fixation of mandibular mentalbody
lamellar fracture, and to analyze the biomechanical changes of mandible under functional loading. Methods A threedimensional
finite element model of mandible was established based on CT data of healthy volunteers. On this model, the
mental-body (right side) laminar fracture of mandible was simulated, and the rigid internal fixation with mini plates was
simulated. According to the length of internal fixation screws, it was divided into single-cortical screw fixation and doublecortical
screw fixation. According to the type of load, it was divided into right-side masticatory load, left-side masticatory
load and bilateral masticatory load, totally 6 operating modes. The displacement between fracture sites and Von Mises stress
distribution of internal fixation plate under each operating mode were analyzed. Results Compared with unicortical screws
fixation operating modes, the displacement between fracture sites in bicortical screws fixation operating modes was shorter. The
displacement between fracture sites reached 0.158 mm when the masticatory forces was applied on the affected side, which was
the longest displacement and exceeded the warning limit of 0.15mm. The stress concentration on internal fixation plate with
bicortical screws was lighter than that with unicortical screws. In addition, the stress concentration on internal fixation plate in
bilateral loading modes was lighter than that in unilateral loading modes. Conclusion For mandibular mental-body lamellar
fractures, the mechanical stability of the mandible fracture fixed with bicortical screws is better than that with unicortical
screws, which is more conducive to creating a better biomechanical environment for bone healing.
