[摘要]目的:探讨大叶桉煎剂浸浴联合短波紫外线照射对深度烧伤残余创面愈合及血清白细胞介素-1β(Interleukin-
1β,IL-1β)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)表达
的作用。方法:选取2017年8月-2019年12月笔者医院收治的烧伤残余创面患者147例采取前瞻性随机对照研究,以计算机生
成的随机数字表分为联合组(n =49)、对照A组(n =49)、对照B组(n =49)。常规治疗基础上,对照A组予以短波紫外线
照射,对照B组予以大叶桉煎剂浸浴,联合组予以大叶桉煎剂浸浴联合短波紫外线照射,均持续治疗2周。比较三组疗效、残
余创面修复情况与治疗前、治疗2周后创面细菌阳性检出率、创面疼痛程度评分(Visual analogue scale,VAS)、焦虑状
况评分(Self-rating anxiety scale,SAS)、血清生长因子指标[表皮生长因子(Epidermal growth factor,EGF)、转
化生长因子-α(Transforming growth factor-α,TGF-α)]、IL-1β、TNF-α、IL-6表达情况。结果:联合组治疗2周
后总有效率高于对照A组及对照B组,联合组创面回缩率、肉芽组织生长良好率高于对照A组及对照B组,创面愈合时间、肉芽
组织生长时间短于对照A组及对照B组,差异有统计学意义(P <0.05)。三组治疗2周后创面细菌阳性检出率较治疗前降低,
且联合组低于对照A组及对照B组,差异有统计学意义(P <0.05)。三组治疗2周后VAS、SAS评分较治疗前降低,且联合组低
于对照A组及对照B组,差异有统计学意义(P <0.05)。三组治疗2周后血清EGF、TGF-α水平较治疗前增高,且联合组高于
对照A组及对照B组,差异有统计学意义(P <0.05);3组治疗2周后血清IL-1β、TNF-α、IL-6水平较治疗前降低,且联合
组低于对照A组及对照B组,差异有统计学意义(P <0.05)。结论:大叶桉煎剂浸浴联合短波紫外线照射治疗深度烧伤残余
创面能通过消除细菌,缓解炎症反应,提高血清生长因子水平,促进创面愈合,减轻疼痛程度,效果显著。

Abstract:
Abstract: Objective To investigate the effect of bathing with Eucalyptus grandis decoction combined with short-wave
ultraviolet irradiation on wound healing and expression of serum interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) and
interleukin-6 (IL-6) in deep burn residual wounds. Methods A total of 147 patients with residual burn wounds in our hospital
from August 2017 to December 2019 were selected for a prospective randomized controlled study, and they were divided into
the combined group (n =49), the control group A (n =49), and the control group B (n =49) using a computer-generated random
number table. On the basis of conventional treatment, the control group A was given short-wave ultraviolet irradiation, the
control group B was given a bath with Eucalyptus grandis decoction, and the combined group was given a bath with Eucalyptus
grandis decoction combined with short-wave ultraviolet irradiation, all of which continued treatment for 2 weeks. Compared
the effi cacy, residual wound repair status, the positive detection rate of bacteria in the wound and the Visual analogue scale
(VAS), Self-rating anxiety scale (SAS), and serum growth factor indicators [Epidermal growth factor (EGF), Transforming
growth factor-α (Transforming growth factor-α, TGF-α)], IL-1β, TNF-α, IL-6 expression before and after treatment for 2 weeks
of the three groups. Results The total effective rate of the combined group after 2 weeks of treatment was higher than that
of the control group A and the control group B, the differences were statistically signifi cant (P <0.05). The wound shrinkage
rate and growth rate of granulation tissue in the combined group were higher than those in the control group A and the control
group B, and the wound healing time and growth time of granulation tissue were shorter than those in the control group A and
the control group B (P <0.05). After 2 weeks of treatment in the three groups, the positive detection rate of wound bacteria was
lower than before treatment, and the combined group was lower than the control group A and the control group B (P <0.05).
After 2 weeks of treatment, the VAS and SAS scores of the three groups were lower than before treatment, and the combined
group was lower than the control group A and the control group B (P <0.05). The serum EGF and TGF-α levels of the three
groups after 2 weeks of treatment were higher than before treatment, and the combined group was higher than the control group
A and the control group B (P <0.05). The levels of serum IL-1β, TNF-α and IL-6 in the three groups after 2 weeks of treatment
were lower than before treatment, and the combined group was lower than that of the control group A and the control group B
(P <0.05). Conclusion The bath with Eucalyptus grandis decoction combined with short-wave ultraviolet irradiation can treat
the residual wounds of deep burns by eliminating bacteria, alleviating the infl ammatory reaction, increasing the level of serum
growth factors, promoting wound healing, and reducing the degree of pain, and the effect is signifi cant.
