[摘要]目的：评估无托槽隐形矫治技术远移上颌磨牙的临床有效性。增加医生在初期设计治疗方案的可预估性，使其更好地
运用于临床诊疗。方法：计算机检索CBM、CNKI、WanFang Data、VIP、PubMed、EMbase、The Cochrane Library、Web of
Science 和 Scopus 数据库，检索隐形矫治技术远移上颌磨牙的相关文献，最后检索日期为2019年4月10日。文献筛选由
两位研究者独立进行，提取资料并交叉核对，若出现意见不同必须通过讨论解决或交由第三位研究者评判。Meta分析采用
RevMan 5.3软件进行数据分析。结果：最终纳入12篇文献进行前瞻性研究，共165例患者，其中女108例，男57例，平均年龄
24.6岁。①隐形矫治远移上颌磨牙有效率的Meta分析结果显示：上颌第一、二磨牙的远移有效率分别为0.85[0.77,0.93]、
0.86[0.79,0.93]；②上颌第一磨牙平均远移2.2mm时，会伴有1.1mm的压低、0.8mm颊向倾斜、2.6°远中倾斜；上颌第二
磨牙平均远移2.3mm时，会伴有1.3mm的压低、0.9mm颊向倾斜、3.1°远中倾斜；③将获取资料根据是否有二类牵引、隐形
矫治器类型(国产隐形矫治器、隐适美)亚组分析，结果显示：二类牵引组和无二类牵引组上颌第一、二磨牙远移有效率
分别为0.85[0.73,0.98]、0.85[0.75,0.94]和0.84[0.69,0.99]、0.87[0.70,1.05]；隐适美组及国产矫治器组上颌第一
磨牙远移有效率分别为0.85[0.77,0.94]、0.84[0.68,1.00]；隐适美组及国产矫治器组上颌第二磨牙远移有效率分别为
0.86[0.78,0.94]、0.87[0.70,1.03]。结论：无托槽隐形矫治技术能有效远移上颌磨牙，但同时伴随有牙齿压低、颊向倾斜
及远中倾斜三维方向的改变。二类牵引及隐形矫治器类型对远移上颌磨牙均无明显影响。

Abstract:
Abstract:Objective To evaluate the clinical efficacy of maxillary molars distalization with clear Aligners.Increase the
predictability of doctors in designing treatment plans in the early stage, so that they can be better applied to clinical diagnosis
and treatment. Methods CBM, CNKI, WanFang Data, VIP, PubMed, EMbase, The Cochrane Library, Web of Science and
Scopus databases were retrieved by computer, and related literatures on maxillary molars distalization by clear aligners.
The last retrieval date was April 10, 2019. Literature screening was carried out independently by two researchers. The data
were extracted and cross-checked. If there was any disagreement, it was resolved through discussion or submitted to the
third researcher for evaluation. Meta analysis was performed using RevMan5.3 software for data analysis. Results Twelve
prospective studies were finally included. A total of 165 subjects, 108 females and 57 males, with an average age of 24.6 years.
①The results of meta-analysis on the effective rate of maxillary molar distalization with aligners showed that the effective
rate of the first and second maxillary molar was 0.85[0.77,0.93] and 0.86[0.79,0.93], respectively. ②When the maxillary first
molar moves 2.2mm on average, it was accompanied by a depression of 1.1mm, a buccal tilt of 0.8mm, and a distal tilt of 2.6°.
The average distal movement of the maxillary second molar was 2.3mm, accompanied by a depression of 1.3mm, a buccal
tilt of 0.9mm, and a distal tilt of 3.1°. ③The obtained data were analyzed according to whether there was elastic and the type
of clear aligners (invisalign, domestic clear aligners) in the subgroup. The results showed that the effective rate of maxillary
first molar and second molar distalization in the group with elastic and the group without elastic were 0.85 [0.73, 0.98], 0.85
[0.75, 0.94] and 0.84 [0.69, 0.99], 0.87 [0.70, 1.05], respectively. The effective rate of maxillary first molar distalization in the
invisalign group and the domestic clear aligners group was 0.85[0.77,0.94] and 0.84[0.68,1.00], respectively. The effective rates
of maxillary second molar diastalization in the invisalign group and the domestic clear aligners group were 0.86[0.78,0.94] and
0.87[0.70,1.03], respectively. Conclusion Clear aligners can effectively move the maxillary molars in the distal direction,but at
the same time it is accompanied by changes in the three-dimensional direction of tooth depression, buccal tilt, and distal tilt.The
influence of elastic and the type of clear aligners had no significant effect on the maxillary molars.
