[摘要]目的：探究带状疱疹（Herpes zoster，HZ）患者皮损程度与血清免疫球蛋白水平和NOD样受体热蛋白结构域相关蛋白
3（NOD-like receptor pyrin domain containing 3，NLRP3）炎性小体水平的相关性。方法：选取2018年11月-2020年11
月就诊于笔者医院的100例带状疱疹患者，根据皮损程度分为轻度组31例、中度组53例和重度组16例。检测并比较各组免疫
球蛋白A（Immunoglobulin A，IgA）、免疫球蛋白G（Immunoglobulin G，IgG）、免疫球蛋白M（Immunoglobulin M，IgM）水
平；NLRP3、含半胱氨酸的天冬氨酸蛋白水解酶-1（Cysteinyl aspartate specific proteinase-1，Caspase-1）信使核糖核酸
（Messenger ribonucleic acid，mRNA）水平；Pearson相关系数分析皮损程度与IgA、IgG、IgM、NLRP3 mRNA、Caspase-1 mRNA
相关性。结果：与轻度组比较，中度组和重度组IgA、IgG、IgM显著降低，重度组显著低于中度组，差异有统计学意义（P＜0.05）；与
轻度组比较，中度组和重度组NLRP3、Caspase-1 mRNA水平显著升高，重度组显著高于中度组，差异有统计学意义（P ＜0.05）。
患者皮损程度与IgA（r =-0.221、-0.253、-0.437，P ＜0.05）、IgG（r =- 0 . 3 1 4 、- 0 . 3 6 1 、- 0 . 3 9 2 ，P ＜0.05）、I g M
（r = - 0 . 4 3 6 、- 0 . 5 0 4 、- 0 . 6 9 6 ，P ＜ 0 . 0 5 ） 呈负相关， 与N L R P 3 m R N A （r = 0 . 2 3 1、0.261、0.514，P ＜0.05）、
Caspase-1 mRNA（r =0.601、0.371、0.324，P ＜0.05）呈正相关。结论：带状疱疹患者皮损程度与血清免疫球蛋白和NLRP3
炎性小体具有密切的相关性，对临床诊治带状疱疹有着一定的参考价值。

Abstract:
Abstract: Objective To explore the correlation between skin damage and serum immunoglobulin level and NOD-like
receptor pyrin domain containing 3 (NLRP3) inflammasome in herpes zoster patients. Methods A total of 100 patients with
herpes zoster admitted to our hospital from November 2018 to November 2020 were selected and divided into the mild group
31 cases, the moderate group 53 cases and the severe group 16 cases according to the degree of skin lesions. The levels of
immunoglobulin A (IGA), immunoglobulin G (IgG), immunoglobulin M (IgM), NLRP3 and Caspase-1 messenger RNA (mRNA)
were detected and compared in each group. Pearson correlation coefficient was used to analyze the correlation between the
degree of skin lesions and IgA, IgG, IgM, NLRP3 mRNA and Caspase-1 mRNA. Results IgA, IgG and IgM in the moderate
group and the severe group were significantly lower than those in the mild group, the severe group was significantly lower than
in the moderate group, and the differences were statistically significant (P ＜0.05). The NLRP3、Caspase-1 mRNA level of the
moderate group and the severe group was significantly higher than those in the mild group, the severe group was significantly
higher than in the moderate group (P ＜0.05). The degree of skin lesions was negatively correlated with IgA (r =-0.221, -0.253,
-0.437, P ＜0.05), IgG (r =-0.314, -0.361, -0.392, P ＜0.05), IgM (r =-0.436, -0.504, -0.696, P ＜0.05), and positively correlated
with NLRP3 mRNA (r =0.231, 0.261, 0.514, P ＜0.05), Caspase-1 mRNA (r =0.601, 0.371, 0.324, P ＜0.05). Conclusion Herpes
zoster patients have close correlation with serum immunoglobulin and NLRP3 inflammatory corpuscles, which has certain
reference value for clinical diagnosis and treatment of herpes zoster.
