摘要:
目的:探讨拔牙位点保留后骨胶原联合自体软组织瓣植入对前牙拔牙后美学满意度的影响。方法:选择2018年1月-2019年7月笔者医院就诊的84例有前牙拔除需求的患者为研究对象,采用随机数字表法分为42例对照组(Bio-Oss Collagen植入),42例观察组(Bio-Oss Collagen+自体软组织瓣植入)。比较两组患者术后拔牙位点愈合情况及两组患者术前、术后6个月牙槽骨高度、牙槽骨宽度,记录两组患者术前、术后6个月牙槽骨密度、垂直向骨吸收量、唇腭向骨吸收量,术后美学效果、满意度,随访1年,观察植后种植体周围骨高度、宽度吸收情况。结果:术后2周拆线,观察组可见拔牙位点愈合良好,无感染、坏死情况,硬腭带蒂结缔组织移植的成活率为100%,对照组拔牙位点愈合良好,但牙槽嵴呈现不同程度的萎缩。两组患者术前牙槽骨宽度、高度、密度比较,差异无统计学意义(P>0.05);术后6个月,两组牙槽骨宽度、高度、密度均低于术前,观察组牙槽骨高度、密度、宽度均高于对照组,差异均有统计学意义(P<0.05)。术后6个月,观察组垂直向骨吸收量、唇腭向骨吸收量均低于对照组,差异有统计学意义(P<0.05)。术后6个月,观察组美学效果、满意度评分较对照组高,随访1年,观察组种植体周围骨吸收低于对照组,差异有统计学意义(P<0.05)。结论:骨胶原联合自体软组织瓣植入可减缓拔牙后牙槽骨高度、宽度及骨密度的下降,在保留骨量方面具有一定的临床价值,能够有效改善患者牙槽美学效果。
Abstract:
Objective To investigate the effect of bone collagen combined with autogenous soft tissue flap implantation on the aesthetic satisfaction of anterior teeth after extraction. Methods From January 2018 to July 2019, 84 patients with anterior teeth extraction demand were randomly divided into control group (Bio-Oss collagen implantation) and observation group (Bio-Oss collagen + autologous soft tissue flap implantation).To compare the healing of tooth extraction sites between the two groups of patients, compare the alveolar bone height and alveolar bone width of the two groups before and 6 months after the operation, record the alveolar bone density and the 6 months after the operation of the two groups Vertical bone absorption, lip and palate bone absorption; postoperative aesthetic effect and satisfaction, After 1 year follow-up, the bone height and width around the implant were absorbed. Results The suture was removed 2 weeks after the operation. The observation group showed that the extraction site was healed well without infection or necrosis. The survival rate of hard palate pedicle connective tissue transplantation was 100%. The tooth extraction site in the control group healed well, but the alveolar ridge showed varying degrees of atrophy. There was no significant difference in the width, height and density of the alveolar bone between the two groups of patients before operation (P >0.05).The height,density and width of alveolar bone in the observation group were higher than those in the control group 6 months after operation, and the alveolar bone width,height and density of the two groups were higher than those of the control group 6 months after operation,the difference was statistically significant (P <0.05).The amount of vertical bone absorption and lip-palate bone absorption in the observation group were lower than those in the control group at 6 months after operation (P<0.05). After 6 months,the aesthetic effect and satisfaction score of the observation group were higher than those of the control group. After 1 year of follow-up, the bone resorption around the implant in the observation group was lower than that in the control group, the difference was statistically significant (P <0.05). Conclusion Bio-Oss collagen combined with autologous soft tissue flap implantation can slow down the decline of alveolar bone height, width and bone mineral density after tooth extraction, which has certain clinical value in bone preservation, and can effectively improve the aesthetic effect of alveolar bone.
