目的：分析无托槽隐形矫治的临床效果及患者接受度的相关影响因素。方法：回顾性选取2017年1月-2020年1月在笔者医院进行牙齿矫治的170例患者为研究对象，根据矫治方法的不同分为隐形组和传统组，每组85例。传统组接受传统固定矫治技术治疗，隐形组接受无托槽隐形矫治技术治疗。回顾性收集两组的临床资料，记录两组矫治时间、矫治前后的牙周健康指数，并以多因素Logistic回归分析方法分析患者对无托槽隐形矫治器治疗接受度的相关影响因素。结果：隐形组倾斜牙竖直、扭转牙矫正的时间长于传统组（P＜0.05），但两组总治疗时间比较，差异无统计学意义（P＞0.05）。矫治后，两组GI、PLI、SBI均显著高于矫治前，但隐形组低于传统组（P＜0.05）；而SPD水平下降，且隐形组低于传统组（P＜0.05）。多因素Logistic回归分析结果显示：家庭月收入＞6000元/月、医生信任度良好均为患者无托槽隐形矫治器治疗接受度的促进因素（OR=3.459，2.273，P＜0.05）。结论：无托槽隐形矫治技术倾斜牙竖直、扭转牙矫正时间延长，可有效改善患者牙周健康指数，但患者家庭月收入、对医生的信任度等因素可能影响其对无托槽隐形矫治器治疗的接受度。
Abstract:
Objective To analyze the clinical effect of bracket-free invisible correction and the related influencing factors of patients’ acceptance. Methods A total of 170 patients undergoing periodontal treatment in our hospital from January 2017 to January 2020 were retrospectively selected and divided into the invisible group and the traditional group according to different treatment methods, with 85 cases in each group. The traditional group received traditional fixed orthodontic treatment, while the invisible group received bracket-free invisible orthodontic treatment. The clinical data of the two groups were collected retrospectively, the treatment time and periodontal health indexs before and after treatment were recorded, and the related influencing factors of patients’ acceptance of bracketless invisible appliance were analyzed by multivariate Logistic regression analysis. Results In the invisible group, the time of righting inclined teeth and twisting displaced teeth was longer than that in traditional group (P ＜0.05), but there was no significant difference in total treatment time between the two groups (P＞0.05). After correction, GI, PLI and SBI in the two groups were significantly higher than those before treatment, but the invisible group was lower than the traditional group (P＜0.05). The level of SPD decreased, and the level of SPD in invisible group was lower than that in traditional group (P＜0.05). Multivariate Logistic regression analysis showed that the monthly family income ＞6 000 yuan/month and good doctor’s trust were all the promoting factors of patients’ acceptance of bracketless invisible appliance (OR=3.459,2.273, P＜0.05）. Conclusion The righting time of tilted teeth and the twisting time of displaced teeth in bracket-free invisible orthodontic treatment is prolonged, but it can effectively improve the periodontal health index of patients. However, the monthly income of patients and their trust in doctors may affect their acceptance of bracket-free invisible appliances.
