摘要:

目的：探究改良全埋入式垂直褥式缝合对二次剖宫产后腹壁切口瘢痕及美观度的影响。方法：选择2019年11月-2020年12月笔者医院妊娠合并瘢痕子宫孕妇60例作为本次研究对象，均行二次剖宫产术，采用随机数字表法分为两组，对照组（n=30，给予传统缝合）及观察组（n=30，给予改良全埋入式垂直褥式缝合），两组均给予相同的红外线照射。比较两组切口缝合时间、切口愈合时间、手术时间、术后美容效果，采用视觉模拟评分（Visual analogue scale，VAS）评估术后1周、1个月疼痛程度，采用温哥华瘢痕量表（Vancouver scar scale，VSS）评估术后1个月、6个月瘢痕严重程度，对比增生性瘢痕发生率。结果：两组切口缝合时间、手术时间比较差异无统计学意义（P＞0.05）；观察组切口愈合时间较对照组短，差异有统计学意义（P＜0.05）。观察组美容总有效率为90.00%，对照组为70.00%，差异有统计学意义（P＜0.05）。观察组术后1周、1个月VAS评分均较对照组低，切口愈合等级情况均优于对照组，差异有统计学意义（P＜0.05）。观察组术后1个月、6个月VSS评分较对照组低，且增生性瘢痕发生率低于对照组，差异有统计学意义（P＜0.05）。结论：改良全埋入式垂直褥式缝合联合红外线照射应用于二次剖宫产产妇，可促进切口愈合，降低增生性瘢痕发生率，提高美容效果。

Abstract:

Objective To explore the effect of modified fully embedded vertical mattress suture on abdominal wall incision scar and aesthetic degree after secondary cesarean section. Methods 60 pregnant women with pregnancy complicated with uterine scar in our hospital from November 2019 to December 2020 were selected as the subjects of this study, all of whom underwent secondary cesarean section. They were divided into two groups by random number table method, respectively as the control group (n=30, conventional suture) and the observation group (n=30, modified fully embedded vertical mattress), both groups were given the same infrared radiation. The incision suture time, incision healing time, operation time and postoperative cosmetic effect were compared between the two groups. The pain degree at 1 week and 1 month after operation was evaluated by Visual analogue scale (VAS), the scar severity at 1 month and 6 months after operation was evaluated by Vancouver scar scale (VAS), and the incidence of hypertrophic scar was compared. Results There was no significant difference in incision sutur e time and operation time between the two groups (P＞0.05). The wound healing time of the observation group was shorter than that of the control group, and the difference was statistically significant (P＜0.05). The total effective rate of cosmetology in the observation group was 90.00%, and that in the control group was 70.00%, the difference was statistically significant (P＜0.05). The VAS score of the observation group at 1 week and 1 month after operation was lower than those of the control group, and the grade of incision healing was better than those of the control group (P＜0.05). The VSS score of the observation group at 1 month and 6 months after operation was lower than those of the control group, and the incidence of hypertrophic scar was lower than that of the control group (P＜0.05). Conclusion The application of modified fully embedded vertical mattress suture combined with infrared irradiation in the second cesarean section can promote the wound healing, reduce the incidence of hypertrophic scar and improve the cosmetic effect.
