目的：探讨心形美容缝合对二次剖宫产产妇切口愈合及瘢痕形成的影响。方法：选取2020年7月-2021年3月于医院妇产科就诊的100例二次剖宫产产妇作为研究对象，采用随机数字表法将其随机分为观察组和对照组，每组50例。两组均行剖宫产术，对照组采用常规美容缝合法，观察组采用心形美容缝合法。随访6个月，比较两组切口缝合时间、切口愈合时间和愈合等级，观察术前和术后1、3、6个月的瘢痕宽度、厚度和温哥华瘢痕量表（Vancouver scar scale，VSS）评分，并比较两组产妇满意度和切口不良反应发生情况。结果：观察组切口缝合时间和愈合时间均低于对照组，切口甲级愈合率高于对 照组，差异有统计学意义（P＜0.05）。术后1、3和6个月，观察组瘢痕厚度、宽度和VSS评分均低于对照组，且产妇满意度明显高于对照组，差异有统计学意义（P＜0.05）。术后2周，观察组切口不良反应发生率为4.00%明显低于对照组16.00%，差异有统计学意义（P＜0.05）。结论：心形美容缝合应用于二次剖宫产产妇能明显减小瘢痕厚度和宽度，改善切口愈合质量，且操作简便，产妇满意度更高。
Abstract:
Objective To explore the effect of heart-shaped cosmetic suture on the wound healing and scar formation in the second cesarean section. Methods 100 cases of second cesarean section in our hospital from July 2020 to March 2021 were selected as the research objects. They were randomly divided into the observation group and the control group with 50 cases in each group. Cesarean section was performed in both groups. Routine cosmetic suture was used in the control group and heart -shaped cosmetic suture was used in the observation group. The patients were followed up for 6 months. The wound suture time, wound healing time and healing grade were compared between the two groups. The scar width, thickness and VSS score before and 1, 3, 6 months after operation were observed. The maternal satisfaction and the incidence of wound adverse reactions were compared between the two groups. Results The suture time and healing time of the incision in the observation group were lower than those in the control group, and the grade A healing rate of the incision was higher than that in the control group, the differences were statistically significant (P＜0.05). At 1, 3 and 6 months after operation, the scar thickness, width and VSS score of the observation group were lower than those of the control group, and the maternal satisfaction was significantly higher than that of the control group (P＜0.05). Two weeks after operation, the incidence of adverse reactions in the observation group was 4.00%, significantly lower than that in the control group (16.00%, P＜0.05). Conclusion The application of heart-shaped cosmetic suture in the second cesarean section can significantly reduce the thickness and width of scar, improve the quality of incision healing, and it is easy to operate with higher maternal satisfaction.
