摘要:
目的:探讨微小切口进路修复颌面部骨折的疗效、咬合功能及不良反应。方法:选择2019年2月-2020年3月笔者医院 收治的60例颌面部骨折患者,均接受微小切口进路内固定修复法。分析治疗后临床疗效、术后1个月骨折愈合情况、咬合功 能恢复情况及并发症发生情况。结果:60例颌面部骨折患者经治疗后,康复36例,好转18例,总有效率为90.00%;张口度恢 复时间为(5.45±1.05)周,软组织消肿时间为(4.25±1.03)d,骨痂形成时间为(4.17±0.17)周;术后,两组最大咬 合力、最大咬合力面积较术前明显增大,咬合力不对称指数较术前明显下降(P<0.05);面神经损伤1例,牙根损伤2例,涎 瘘2例,感染1例,无开口受限患者,总并发症发生率为10.00%。结论:微小切口进路对颌面部骨折疗效显著,骨折愈合迅 速,咬合功能改善显著,且副作用少,值得临床推广应用。
Abstract:
Objective To investigate the efficacy, occlusion function and adverse reactions of micro-incision approach for internal fixation of maxillofacial fractures. Methods 60 patients with mandibular fracture in our hospital from February 2019 to March 2020 were enrolled, and received micro-incision and internal fixation. The clinical efficacy, fracture healing at one month after operation, recovery of occlusal function, and the occurrence of complications were compared between the two groups. Results TAfter treatment, 36 cases recovered and 18 cases improved, the total effective rate was 90.00%.The time of opening recovery was(5.45±1.05)weeks, soft tissue swelling time was (4.25±1.03) d, callus formation time was (4.17±0.17) weeks. After treatment, the area and maximum bite force of the two groups were significantly increased, and the asymmetry index of biting force was significantly decreased (P<0.05). There were 1 case of facial nerve injury, 2 cases of tooth root injury, 2 cases of salivary fistula, 1 case of infection, and the total incidence of complications was 10.00%. Conclusion Micro-incision approach is more effective in the treatment of maxillofacial fractures, with rapid fracture healing, significant improvement of occlusal function and less side effects, which is worthy of clinical application.
