摘要:
目的:研究牙周加速成骨正畸(Periodontally accelerated osteogenic orthodontics,PAOO)对关闭下颌第一磨 牙间隙的时间及下颌第二磨牙牙根吸收量的影响。方法:选取42例下颌第一磨牙缺失或因龋无法保留的成年患者,随机 分为PAOO辅助治疗组(A组,21例)及常规正畸治疗组(B组,21例)。矫治前后拍摄锥形束CT(Cone beam computer tomography,CBCT),测量两组患者排齐时间、关闭间隙时间及下颌第二磨牙近远中根吸收量,比较两组之间差异。结果: A、B组排齐时间相比差异无统计学意义(P>0.05)。A组关闭间隙时间为(9.87±0.92)个月,B组为(16.79±1.67)个月,两 组差异有统计学意义(P<0.01)。A组排齐与关闭间隙总时间为(16.31±1.18)个月,B组为(23.41±1.99)个月,两组 差异有统计学意义(P<0.01);A组与B组矫治后近远中根均有一定程度吸收,A组近中根吸收量为(0.61±0.34)mm,B组 为(1.46±0.57)mm;A组远中根吸收量为(0.56±0.29)mm,B组为(1.62±0.45)mm,两组近远中根吸收量对比差异均有统计学意义(P<0.01)。结论:PAOO可以加速下颌第二磨牙的近中移动,缩短正畸治疗时间,并减少其牙根吸收。
Abstract:
Objective To investigate the eff ect on the time of closing space of mandibular fi rst molar and the root resorption of mandibular second molar by Periodontally accelerated osteogenic orthodontics (PAOO). Methods 42 adult patients with mandibular fi rst molar missing or unable to retain due to caries were randomly divided into PAOO assisted treatment group (group A, 21 cases) and conventional orthodontic treatment group (group B, 21 cases). CBCT was taken before and after orthodontic treatment. The alignment time,closing space time and mesial and distal root resorption of mandibular second molar were measured. The diff erences between these two groups were compared. Results There was no signifi cant diff erence between group A and group B in alignment stage(P>0.05). Time needed for space closure of group A was (9.87±0.92) months, group B was (16.79±1.67) months, the diff erence between these two groups was statistically signifi cant(P<0.01).Time needed for aligning and spacec closure of group A was (16.31±1.18) months, group B was (23.41±1.99) months,the diff erence between these two groups was statistically signifi cant (P<0.01); The mesial and distal roots of group A and group B were absorbed after treatment. In group A, the absorption of mesial root was (0.61±0.34) mm, group B was (1.46±0.57) mm;The absorption of distal root in group A was (0.56±0.29) mm, group B was (1.62±0.45) mm,the diff erence between these two groups was statistically signifi cant(P<0.01). Conclusion PAOO can eff ectively accelerate the mesial movement of mandibular second molar, shorten orthodontic treatment time and reduce root resorption.
