摘要:
目的：探究改良冲洗装置联合标准化疼痛护理对中重度压疮患者治疗效果和生活质量的影响。方法：选取2020年6 月-2021年6月收治的236例中重度压疮患者，按照随机数字表法随机分为对照组（118例）和观察组（118例），对照组采 用常规负压冲洗装置处理创面并进行常规护理，观察组采用改良冲洗装置处理创面并进行标准化疼痛护理，比较两组治疗 效果、疼痛情况[视觉模拟评分法（Visual analogue scale，VAS）]、心理状态[焦虑自评量表（Self-rating Anxiety Scale，SAS）、抑郁自评量表（Self-rating depression scale，SDS）]、创面愈合满意度、生活质量[生活质量综合评定 问卷（Generic quality of life inventory-74，GQOLI-74）]。结果：相较于对照组，观察组患者创面愈合时间、渗液消 失时间缩短，换药次数减少，治疗2周后的压疮愈合评价量表评分降低，差异有统计学意义（P＜0.05）；护理后，两组患者 VAS 评分、SAS评分、SDS评分均降低，且观察组低于对照组，差异有统计学意义（P＜0.05）；观察组患者创面愈合满意度高 于对照组，差异有统计学意义（P＜0.05）；护理后，两组患者生活质量量表中躯体功能、心理功能、社会功能、物质生活 状态维度评分均提高，且观察组高于对照组，差异有统计学意义（P＜0.05）。结论：改良冲洗装置联合标准化疼痛护理有 利于中重度压疮患者创面快速愈合，还可减少换药次数、减轻患者疼痛和心理压力，从而提高患者生活质量。
Abstract:
Objective To explore the influence of modified flushing device combined with standardized pain nursing on the treatment effects and quality of life of patients with moderate-to-severe pressure ulcers. Methods 236 patients with moderate-to-severe pressure ulcers treated in the hospital were selected between June 2020 and June 2021,and were randomly divided into the control group (118 cases) and the observation group (118 cases) according to the random number table method.The control group was given conventional negative pressure fl ushing device to wash wound and underwent routine nursing,while the observation group adopted modified flushing device for wound and received standardized pain nursing.The treatment eff ects,pain status [Visual Analogue Scale (VAS)],psychological status [Self-rating Anxiety Scale (SAS),Self-rating Depression Scale (SDS)],wound healing satisfaction and quality of life [Generic Quality of Life Inventory-74 (GQOLI-74)]were compared between the two groups. Results Compared with control group,the wound healing time and exudation disappearance time in observation group were shortened,and the number of dressing changes was reduced,and the Pressure Ulcer Scale for Healing score was decreased after 2 weeks of treatment (P ＜0.05).After nursing,the VAS score,SAS score and SDS score of the two groups were declined,and the scores in observation group were lower than those in control group (P ＜0.05).The wound healing satisfaction was higher in observation group than that in control group (P＜0.05).After nursing,the scores of dimensions of physical function,psychological function,social function and material life status of quality of life scale were improved in the two groups,and the scores in observation group were higher compared with those in control group (P ＜0.05). Conclusion Modified flushing device combined with standardized pain nursing is beneficial to the rapid wound healing of patients with moderateto-severe pressure ulcers,and can reduce the number of dressing changes and relieve the pain and psychological stress of patients,thus improving the quality of life of patients.
