摘要:

目的：探讨闭合入路保留鼻解剖结构的鼻整形术对就医者术后鼻形态及并发症的影响。方法：选取笔者医院2018年12 月-2020年12月116例行鼻整形术就医者按照随机抽签法分为观察组与对照组，每组58例。对照组采用传统开放入路的鼻整形 术，观察组采用闭合入路保留鼻解剖结构的鼻整形术。统计两组手术时间、术中出血量、术后引流量、愈合时间，测量两组 术前及术后6月鼻额角、鼻面角、鼻尖凸度、鼻唇角，评估两组术后3、6个月满意度评分[鼻整形结果评估量表(Rhinoplasty outcome evaluation,ROE)]，统计两组术后并发症发生情况。结果：观察组手术时间、愈合时间显著短于对照组、术中出血 量及术后引流量显著少于对照组（P＜0.05）。术后6个月两组鼻额角、鼻面角、鼻尖凸度显著高于术前，且观察组高于对照 组（P＜0.05），术后6个月两组鼻唇角显著低于术前，且观察组低于对照组（P＜0.05）。观察组术后3、6个月满意度评分 均显著高于对照组（P＜0.05）。观察组术后3、6个月满意度评分均显著高于对照组（P＜0.05），两组术后6个月满意度评 分均显著高于术后3个月（P＜0.05）。观察组术后并发症发生率显著低于对照组（P＜0.05）。结论：与传统开放入路的鼻 整形术相比，闭合入路保留鼻解剖结构的鼻整形术手术时间及就医者术后恢复时间更短，术中出血量、术后引流量更少，对 鼻背、鼻尖形态塑造效果更好，就医者满意度更高，并发症发生率更低，有临床推广价值。

Abstract:

Objective To investigate the eff ect of closed approach rhinoplasty with nasal anatomical structure on postoperative nasal morphology and complications. Methods 116 patients who underwent rhinoplasty in the author’s hospital from December 2018 to December 2020 were randomly divided into observation group and control group, with 58 cases in each group.The control group used the traditional open approach rhinoplasty,and the observation group used the closed approach to preserve the nasal anatomical structure of rhinoplasty.The operation time, intraoperative bleeding, postoperative drainage volume and healing time of the two groups were counted,The nasal frontal angle, nasal facial angle, nasal tip convexity and nasolabial angle were measured before and 6 months after operation,The satisfaction score [rhinoplasty result evaluation scale (ROE)] at 3 and 6 months after operation was evaluated, and the incidence of postoperative complications was counted. Results The operation time,intraoperative bleeding,postoperative drainage and healing time in the observation group were signifi cantly less than those in the control group (P ＜0.05).6 months after operation,the nasal frontal angle, nasal facial angle and nasal tip convexity in the two groups were signifi cantly higher than those before operation, and that in the observation group was higher than that in the control group (P ＜0.05). 6 months after operation, the nasal lip angle in the two groups was signifi cantly lower than that before operation, and that in the observation group was lower than that in the control group (P ＜0.05).The satisfaction scores of the observation group at 3 and 6 months after operation were signifi cantly higher than those of the control group (P ＜0.05).The satisfaction scores of the observation group at 3 and 6 months after operation were signifi cantly higher than those of the control group (P ＜0.05), and the satisfaction scores of the two groups at 6 months after operation were signifi cantly higher than those at 3 months after operation (P＜0.05).The incidence of postoperative complications in the observation group was signifi cantly lower than that in the control group (P ＜0.05). Conclusion Compared with the traditional open approach rhinoplasty,the closed approach preserving the nasal anatomical structure has less operation time, intraoperative bleeding, postoperative drainage and postoperative recovery time, better shape shaping effect on the nasal back and tip, higher patient satisfaction and lower incidence of complications, which is of clinical value.
