目的:评价柠檬酸联合甘醇酸治疗单纯色素型黄褐斑的有效性及安全性。方法:选取2018年12月-2019年10月就诊于 笔者医院,诊断为色素型黄褐斑的50例患者为研究对象,根据随机数字表法分为试验组和对照组。试验组给予柠檬酸联合甘 醇酸治疗,对照组给予甘醇酸治疗,每次间隔2~3周,共治疗5次。所有患者每次治疗前及末次治疗结束后1个月随访、拍照 并行黄褐斑面积及严重程度(Melasma area severity index,MASI)评分、行VISIA检测,治疗过程进行疼痛评分,术后第 1 、3、5天对患者进行电话随访,记录患者术后出现的不良反应。使用MASI评分、VISIA检测等进行效果评估。结果:试验组 和对照组黄褐斑患者治疗有效率分别为80.00%,68.00%。两组患者经过治疗后,面部黄褐斑均有不同程度改善,MASI评分均 较前降低,差异有统计学意义(P<0.05),且试验组MASI下降程度较对照组明显;VISIA检测提示两组患者治疗后斑点、紫 外线色斑、棕色斑数值均下降,差异具有统计学意义(P<0.05)。两组治疗后无明显不良反应,安全性高,患者主观满意 度高。结论:柠檬酸联合甘醇酸治疗单纯色素型黄褐斑安全、有效,可临床推广应用。 [
Abstract:
Objective To evaluate the effi cacy and safety of citric acid combined with glycolic acid in the treatment of melasma. Methods 50 melanized type melasma patients were selected from the author’s hospital. According to the random comparison table, they were divided into the treatment group and the control group. The treatment group was treated with citric acid combined with glycolic acid, and the control group was treated with glycolic acid only, each at an interval of 2-3 weeks, for a total of 5 treatments.All patients were followed up before each operation and 1 month after the end of the last treatment, melasma area severity index (MASI) score, VISIA detection and photographs to contrast before-after photos to compare the improvement eff ect. Pain scores were scored during the treatment, and patients were followed up by telephone on day 1, 3, and 5 after the treatment, and adverse reactions were recorded. MASI score and VISIA score were used for eff ect evaluation. Results The eff ective rate of two groups weas 80.00% and 68.00%, respectively.Both groups showed improvement in melasma after treatment, MASI score was lower than before. There were statistical signifi cance in MASI scores between the two groups before and after treatment (P <0.05), and the decrease of MASI in the treatment group was more obvious than that in the control group. VISIA detection indicated that there were statistical signifi cance in the values of surface spots, UV spots and brown spots between the two groups after treatment (P <0.05). There was no obvious adverse reaction, high safety and high subjective satisfaction. Conclusion Citric acid in combination with glycolic acid for treatment of melanized type melasma is safe and eff ective,can be applied clinically.
