摘要:
目的:探讨根管治疗术后树脂充填联合固位纤维冠外保护对上颌第一前磨牙牙齿抗折性能的影响。方法:选择2018 年6月-2020年8月笔者医院因正畸拔除的60颗冠根完整的上颌第一前磨牙为研究对象,将60颗离体牙样本分为对照组(3M 流体树脂填充),观察组(3M流体树脂填充联合固位纤维冠外保护),采用游标卡尺测量并比较两组第一前磨牙牙体标本 解剖外形各项指标(牙全长、冠长、颈部颊舌径及近远中径),采用静态加载法和循环加载法对离体牙折裂载荷及离体牙 折裂模式进行测定。结果:两组离体牙体标本解剖外形各项指标比较无统计学意义(P>0.05)。对照组平均折裂载荷为 (627.96±91.42)N,观察组为(974.85±102.25)N,观察组折裂载荷值明显高于对照组;对照组可修复性牙折裂20颗 (66.67%),观察组可修复性牙折裂27颗(90.00%),差异有统计学意义(P<0.05)。对照组中33.33%的离体牙样本表现 为修复体和超过一半的牙体破坏或破坏涉及牙根,无法修补;观察组中10.00%的离体牙样本无法修补,差异有统计学意义 (P<0.05)。结论:3M流体树脂充填辅以固位纤维冠外保护可显著提高根管治疗后患牙的抗折性能。
Abstract:
Objective To investigate the effect of resin filling combined with retention fiber extracoronal protection on the fracture resistance of maxillary fi rst premolar after root canal therapy. Methods 60 maxillary fi rst premolars with complete crown roots extracted from the author’s hospital from June 2018 to August 2020 due to orthodontics were selected as the research objects, and 60 ex vivo tooth samples were divided into control group (3M fl uid resin fi lling) and observation group (3M fl uid resin fi lling combined with fi xed fi ber extracrown protection), and the anatomical appearance indexes (total tooth length, crown length, neck buccal tongue diameter and near and far middle diameter) of the two groups of fi rst premolars were measured and compared by vernier calipers. The static loading method and cyclic loading method were used to determine the fracture load of ex vivo teeth and the fracture mode of ex vivo teeth. Results There was no signifi cant signifi cance in the anatomical appearance of the two groups of ex vivo tooth specimens (P >0.05). The average fracture load in the control group was (627.96±91.42)N, and the observation group was (974.85±102.25) N, and the fracture load value in the observation group was signifi cantly higher than that in the control group. There were 20 (66.67%) restorable tooth fractures in the control group and 27 (90.00%) fractures in the observation group, with a statistically signifi cant diff erence (P <0.05). 33.33% of the ex vivo tooth samples in the control group showed restoration and more than half of the tooth damage or destruction involved the tooth root and could not be repaired; 10.00% of the ex vivo tooth samples in the observation group could not be repaired, and the diff erence was statistically signifi cant (P<0.05). Conclusion 3M fl uid resin fi lling combined with retention fi ber crown protection can signifi cantly improve the fracture resistance of aff ected teeth after root canal treatment.
