目的：观察基于行为转变理论即行为阶段转变理论模型（The transtheoretical model and stages of change， TTM ）的阶段性护理模式在四肢烧伤康复期患者中的应用效果。方法：选取2019年1月-2022年3月笔者医院收治的145例处于 康复期的肢体烧伤患者，按随机奇偶编号分组，将奇数编号患者纳入观察组（73例），偶数编号患者纳入对照组（72例）。 对照组予以常规护理，观察组在对照组护理的基础上予以基于行为转变理论的阶段性护理。两组均持续干预3个月，比较两 组患者瘢痕状况、心理状态、自我效能、社会支持情况及患者对护理的满意度。结果：干预后，观察组患者瘢痕及疼痛评分 （Vision algetic standard，VAS）均低于对照组（P＜0.05）；观察组心理韧性评估量表（Connor-davidson resilience scale ，CD-RISC）评分高于对照组，中文版体象障碍自评量表（Chinese version of the body image disorder selfrating scale ，BIRS-C）评分低于对照组，差异均有统计学意义（P＜0.05）；观察组自我效能感量表（General selfefficacy scale ，GSES）及心理特征应对量表（Trait coping style questionnaire，TCSQ）-积极评分高于对照组， TCSQ- 消极评分低于对照组，差异均有统计学意义（P＜0.05）；观察组患者主观支持、客观支持及社会利用度均高于对照 组（P＜0.05）。观察组对沟通交流、专业指导、心理护理等满意度评分均高于对照组（P＜0.05）。结论：基于TTM的阶段 性护理可有效提高患者心理韧性及自我效能，改变其消极态度，降低其创面疼痛感及体象感知偏差，提高其社会支持程度， 提升患者康复效果，且可有效提高患者整体护理质量。
Abstract:
Objective To observe the application effect of staged nursing based on transtheoretical model and stages of change (TTM) in patients with limbs burn during rehabilitation period. Methods A total of 145 patients with limb burns during rehabilitation period admitted to the hospital were enrolled between January 2019 and March 2022. According to oddeven number method, they were divided into observation group (73 cases, odd number) and control group (72 cases, even number). The control group was given routine rehabilitation nursing,The observation group was given staged nursing based on behavioral change theory on the basis of the control group. All were continuously intervened for 3 months. The scars recovery, psychological state, self- efficacy and social support were compared between the two groups.The satisfaction evaluation of diff erent nursing modes was compared between the two groups. Results After intervention, scores of scars and visual analogue scale (VAS) in observation group were lower than those in control group (P ＜0.05). The score of Connor-Davidson resilience scale (CD-RISC) in observation group was higher than that in control group, and score of Chinese version of the Body image Disorder Self-rating Scale (BIRS-C) was lower than that in control group,the differences were all statistically significant (P ＜0.05). The scores of General Self-efficacy Scale (GSES) and Trait Coping Style Questionnaire (TCSQ)-positive in observation group were higher than those in control group, and score of TCSQ-negative was lower than that in control group,the diff erences were all statistically signifi cant (P ＜0.05). The scores of subjective support, objective support and social utilization in observation group were higher than those in control group group (P ＜0.05). The scores of communication, professional guidance and psychological nursing in observation group were higher than those in control group (P＜0.05). Conclusion The staged nursing based on TTM can effectively improve psychological resilience and self-efficacy of patients, change negative attitude, reduce wound pain and body image perception deviation, improve social support,rehabilitation eff ect,and can eff ectively improve the overall quality of nursing.
