目的：探讨新型强脉冲光（Advanced optimal pulsed technology，AOPT）联合表皮干细胞外泌体（Extracellular vesicles released from epithelial stem cells，EVS）治疗黄褐斑的临床疗效及安全性。方法：选2019年3月-2021年12 月在笔者医院接受治疗的89例黄褐斑患者，随机分为实验组（AOPT+EVS治疗组）和对照组（AOPT治疗组），疗程24周，比 较治疗前后黄褐斑面积和严重指数（Melasma area severity index，MASI）评分，医生整体评价（Physician’s global assessment，PGA）、患者满意度及出现的不良反应。结果：实验组治疗后MASI评分由（18.1±8.0）分下降至（4.2±3.6） 分；对照组治疗后MASI评分由（17.6±6.4）分下降至（5.8±3.8）分，两组治疗前后差异有统计学意义（P＜0.05），两组 间治疗前后差值比较差异有统计学意义（P＜0.05）。两组PGA评分均有改善，实验组改善率97.7%，对照组改善率93.3%；患 者满意度实验组88.6%，高于对照组的80.0%。治疗期间不良反应轻微，未观察到严重不良反应。结论：新型强脉冲光治疗黄 褐斑疗效确切，术后配合EVS外用可以在一定程度上提高治疗效果。
Abstract:
Objective To investigate the clinical effi cacy and safety of Advanced Optimal Pulsed Technology (AOPT) combined with extracellular vesicles released from epithelial stem cells (EVS) in the treatment of melasma. Methods A total of 89 patients with melasma who were treated in author’s hospital from March 2019 to December 2021, were randomized to the experimental group (AOPT+EVS) and the control group (AOPT), with a treatment course of 24 weeks. The evaluations were melasma area and severity index (MASI) score before and after treatment, physician’s global assessment (PGA), patient satisfaction and adverse reactions. Results The MASI score of the experimental group decreased from (18.1±8.0) to (4.2±3.6) after treatment; while that of the control group decreased from (17.6±6.4) to (5.8±3.8) after treatment. The diff erence between before and after treatment was statistically signifi cant (P ＜0.05). There was a signifi cant diff erence between two groups for the change of MASI score after treatment (P ＜0.05). The improvement rate of PGA in the experimental group was 97.7% vs 93.3% in the control group. Patient satisfaction was 88.6% in the experimental group, which was higher than 80.0% in the control group. The adverse reactions were mild, and no serious adverse reactions were observed during the treatment. Conclusion AOPT was eff ective in the treatment of melasma, and external use of EVS after surgery can improve the treatment eff ect to a certain extent.
