摘要:
目的：研究经结膜入路眶隔脂肪带蒂移植与游离移植两种不同的移植方法对睑袋伴泪沟凹陷就医者泪沟凹陷改善和美 观满意度的影响。方法：选取2019年1月-2022年1月笔者医院收治的睑袋伴泪沟凹陷就医者160例，按随机数字表法分为研 究组与对照组各80例。两组就医者均行结膜入路睑袋整复术，其中，对照组就医者在术中采用眶隔脂肪游离移植矫正 泪沟凹陷，研究组在术中采用眶隔脂肪释放带蒂移植矫正泪沟凹陷。比较两组临床疗效、手术前后泪沟畸形等级量表 （Tear trough rating scale，TTRS）评分、术后就医者美观满意度及肿胀、溢泪、结膜出血、局部结节等并发症 发生情况。结果：术后6个月，研究组总有效率97.50%高于对照组的87.50%（P＜0.05）；研究组就医者TTRS评分低 于对照组（P＜0.05）；研究组就医者美观满意度达到97.50%，高于对照组的88.75%（P＜0.05）；研究组就医者并发症总 发生率5.00%低于对照组的15.00%（P＜0.05）。结论：行结膜入路睑袋整复术时，采用眶隔脂肪释放带蒂移植矫正泪沟凹陷 效果确切，能有效改善睑袋伴泪沟凹陷就医者的泪沟凹陷程度，术后就医者美观满意度高，且能减少术后并发症的发生。
Abstract:
Objective The purpose of this study was to investigate the eff ects of pedicle orbital septum fat transplantation and free orbital septum fat transplantation via transconjunctival approach on lacrimal groove depression and aesthetic satisfaction in patients with pouch and lacrimal groove depression. Methods A total of 160 patients with pouch and lacrimal groove depression were treated in the hospital from January 2019 to January 2022. They were selected as the research subjects, and assigned to a study group and a control group by random number table method, with 80 patients in each group. All patients were treated with transconjunctival lower eyelid blepharoplasty. For the control group, free orbital septum fat grafts were used to correct lacrimal groove depression, while for the study group, lacrimal groove depression was corrected by pedicle orbital septum fat transplantation. Comparison was made between the two groups in terms of clinical eff ect, the tear trough rating scale (TTRS) scores before and after surgery, postoperative aesthetic satisfaction, and complications. Results Six months after surgery,the total clinical response rate in the study group was 97.50%, higher than 87.50% in the control group (P ＜0.05). The TTRS scores of the study group after surgery were lower than those of the control group (P＜0.05). The aesthetic satisfaction rate in the study group was 97.50%, higher than 88.75% in the control group (P＜0.05). The total incidence of complications in the study group was 5.00%, lower than 15.00% in the control group (P＜0.05). Conclusion Applying pedicle orbital septum fat transplantation to correct lacrimal groove depression during transconjunctival lower eyelid blepharoplasty can eff ectively improve the degree of lacrimal groove depression in patients with pouch and lacrimal groove depression, improve their postoperative aesthetic satisfaction, and can reduce postoperative complications.
