摘要:
目的：分析手术切除联合自体断层皮片移植治疗面部鳞状细胞癌（Squamous cell carcinoma，SCC）的临床疗效。方 法：选择2019年1月-2021年1月笔者医院收治的53例面部SCC患者，按入院顺序依随机数字表法分组，对照组（n=26）给予 手术切除联合常规外科修复，研究组（n=27）给予手术切除联合自体断层皮片修复。比较两组临床疗效、血管内皮生长因 子（Vascular endothelial growth factor，VEGF）与转化生长因子β1（Transforming growth factor-β1，TGF-β1）水 平、美学效果、创面愈合时间及肿瘤复发情况。结果：术后14 d，研究组总有效率高于对照组（P＜0.05）；术后7 d，研究 组VEGF与TGF-β1水平均高于对照组（P＜0.05）；出院时，研究组美学效果优于对照组（P＜0.05）；研究组创面愈合时间 短于对照组（P＜0.05），术后1年内复发率低于对照组（P＜0.05）。结论：手术切除联合自体断层皮片移植治疗面部SCC创 面愈合时间短，美学效果好，复发率低，整体疗效肯定。
Abstract:
Objective To analyze the clinical effi cacy of surgical resection combined with autologous tomographic skin grafting in the treatment of facial squamous cell carcinoma (SCC). Methods 53 patients with facial SCC accepted by the author’s hospital from January 2019 to January 2021 were collected and separated into groups according to the random number table method.The control group (n=26) received surgical resection combined with conventional surgical repair,and the research group (n=27) received surgical resection combined with autologous tomographic skin grafting.The clinical effi cacy,vascular endothelial growth factor (VEGF) and transforming growth factor-β1(TGF-β1) levels,aesthetic eff ects,wound healing time,and neoplasm recurrence were compared. Results At 14 days after operation,the total eff ective rate of the study group was higher than that of the control group (P ＜0.05). At 7 days after operation,the levels of VEGF and TGF-β1 in the study group were higher than those in the control group (P ＜0.05). At discharge,the aesthetic eff ect of the study group was better than that of the control group (P ＜0.05). The wound healing time of the study group was shorter than that of the control group (P＜0.05),and the recurrence rate within 1 year after operation was lower than that of the control group (P＜0.05). Conclusion Surgical resection combined with autologous split-thickness skin grafting in the treatment of facial SCC has the advantages of short wound healing time, good aesthetic eff ect, low recurrence rate,and positive overall effi cacy.
