摘要:
目的：探讨男跨女性别重置术中外阴皮下组织在重塑女性外阴时的作用。方法：100例男跨女性别重置术中，充分保 留外阴结缔组织，包括精索外结缔组织，海绵体外脂肪组织及阴囊肉膜组织，并将这些组织分位置、分层次的填充于再造 外阴，对外阴进行塑形。结果：所有患者术后14～25 d出院，再造大阴唇皮瓣无明显坏死，术区肿胀1个月内基本消失；术 后再造大阴唇皮下组织存在不同程度的萎缩，3个月时基本稳定。术后随访6个月，患者再造大阴唇形态自然、逼真，触感 柔软、均匀，无明显凹凸及硬结；患者自我感受良好，性兴奋时无明显不适感。90例患者对再造外阴满意，4例患者基本满 意，6例患者再造大阴唇皮肤由于消肿出现皮肤褶皱，下极凹陷，经过二期修复取得满意效果。结论：利用阴囊肉膜等组织 代替女性大阴唇丰富的皮下脂肪组织，对再造大阴唇进行塑形，再造大阴唇形态逼真，触感柔软、均匀，患者满意度高，返 修率低，是一种理想的手术方式，值得借鉴、推广。
Abstract:
Objective To explore the role of the male vulva subcutaneous tissue in the reconstruction of female vulva. Methods In 100 cases of male-to-female gender reassignment,fully retained the vulvar connective tissue include external cord connective tissue,extracavernous body adipose tissue and scrotal fl esh membrane tissue,and fi lled these tissues in positions and layers to Shape the vulva,especially the large labia. Results All patients were discharged from hospital 14 to 25 days after operation. There was no obvious necrosis of the reconstructed labia majora flaps,and the swelling of the operation area disappeared within 1 month. The subcutaneous tissue of the reconstructed labia majora was atrophic to varying degrees,but it was basically stable at 3 months after operation. After 6 months of follow- up,the shape of reconstructed labia majora was natural and realistic, the touch was soft and uniform,without obvious bumps and induration. The patient felt good about himself and had no obvious discomfort during sexual excitement. Ninety patients were satisfi ed with the reconstructed vulva,4 patients were basically satisfi ed with the reconstructed vulva,and 6 patients had skin folds and lower pole depression due to swelling in the reconstructed labia majora. After the second stage repair,satisfactory results were obtained. Conclusion Using scrotal meat membrane and other tissues to replace the rich subcutaneous adipose tissue of the female labia majora to shape the reconstructed labia majora is realistic,soft and uniform,with high patient satisfaction and low repair rate. It is an ideal surgical method,which is worthy of reference and promotion.
