摘要:
目的:探讨一种由硅胶导尿管、一次性吸引管及20 ml一次性注射器制作的负压引流装置在扩张法耳再造术后的临床 应用效果。方法:收集2021年1月-2022年1月笔者医院整形外科收治的扩张法耳再造患者19例,采用该装置进行术后引流, 观察其引流效果,负压调整次数,吸闭次数,术后并发症情况,耳廓与亚结构的显现情况。结果:临床小实验结果显示,两 次抽吸引流管内空气后,普通一次性引流管变软,略有变形;输液器引流管完全吸闭;硅胶导尿管无变形吸闭现象。硅胶导 尿管作为引流管引流通畅,皮瓣血运良好,耳廓形态满意无一例出现吸闭、感染现象,术区伤口均为一期愈合,平均日引流 量为8~17 ml,拔管时间为4~7 d。结论:该装置既能使皮瓣与耳支架紧密贴合,保证再造耳满意形态,又能充分引流,实 现零吸闭,其抗菌抗感染的材质及双腔结构还能减少感染概率并且提供预防性使用抗生素冲洗的机会。
Abstract:
Objective To investigate the clinical application effect of a negative pressure drainage device made of silicone catheter,disposable suction tube and 20ml disposable syringe after dilated ear reconstruction. Methods From January 2021 to January 2022,30 patients with expanded ear reconstruction in the author’s hospital were collected. The drainage eff ect, negative pressure adjustment times, suction and closure times,postoperative complications,and auricle and substructure development were observed. Results The results of a small clinical trial show that after two times of air suction in the fl ow tube, the ordinary disposable drainage tube became soft and slightly deformed. Infusion set drainage tube completely suction closed. The silicone catheter has no deformation and suction phenomenon. The silicone catheter was used as a drainage tube, the blood supply of the fl ap was good, the substructure of the auricle was satisfactory, and there was no phenomenon of aspiration closure and infection. All incisions healed by fi rst intention.The average drainage volume of the wound was 8 ~17ml daily, and the time to extubation was from 4 ~7 days. Conclusion The device can not only make the fl ap fi t closely with the ear scaff old, ensure the satisfactory shape of the reconstructed ear, but also fully drain the ear to achieve zero suction closure. Its antibacterial and anti-infection material and double cavity structure can also reduce the incidence of infection and provide the opportunity of prophylactic antibiotic irrigation.
