摘要:
目的：探究iRoot SP 单尖法根管充填治疗糖尿病合并慢性根尖周炎患者的疗效及美学效果。方法：选取2020年8 月-2022年12月笔者医院收治的120例糖尿病合并慢性根尖周炎患者，采用数字化随机法将其分为观察组和对照组，每组60 例。对照组采用AH Plus+热牙胶垂直加压技术充填治疗，观察组采用iRoot SP 单尖法根管充填治疗。观察比较两组患者根 管充填时间、充填后1 d、3 d、5 d和7 d的疼痛VAS评分，充填前后根尖周指数(Periapical index,PAI)、充填后3个月的临床疗 效，充填前后患者红色美学指数评分（Pink esthetic score,PES）和白色美学指数评分（White esthetic score,WES），充填 前后患者的菌斑指数（Plaque index,PLI）、牙周袋探诊深度（Probing depth,PD）和龈沟出血指数（Sulcus bleeding index,SBI）。 结果：对照组充填单位根管时间较观察组长，充填后1周内观察组患者PAI指数低于对照组，且充填后1 d、3 d、5 d、7 d的VAS评分低 于对照组（P＜0.05）；充填后3个月，观察组的总有效率与对照组相当，差异无统计学意义（P＞0.05）；充填前，两组患者PES 和WES评分比较，差异无统计学意义（P＞0.05），充填后3个月，观察组患者PES和WES评分均高于对照组，PLI、PD及SBI均 低于对照组（P＜0.05）。结论：相较AH Plus+热牙胶垂直加压技术，iRoot SP 单尖法充填治疗糖尿病慢性根尖周炎患者的 充填效率更高，充填后疼痛感更轻，成功率更高，美学效果更显著，值得临床推广应用。
Abstract:
Objective To explore the curative eff ect and aesthetic eff ect of iRoot SP single apex root canal fi lling in patients with diabetes mellitus and chronic periapical periodontitis. Methods A total of 120 patients with diabetes mellitus and chronic periapical periodontitis admitted to the author’s hospital were enrolled between August 2020 and December 2022. According to random grouping method, they were divided into observation group and control group, 60 cases in each group. The control group was treated with AH Plus+ warm vertical condensation, while observation group was treated with iRoot SP single apex root canal fi lling. The root canal fi lling time, VAS scores at 1 d, 3 d, 5 d and 7 d after fi lling, periapical index (PAI) before and after fi lling, clinical curative eff ect at 3 months after fi lling, pink esthetic score (PES) and white esthetic score (WES) before and after fi lling, plaque index (PLI), probing depth (PD) and sulcus bleeding index (SBI) in the two groups were observed and compared. Results The fi lling time of unit root canal in control group was longer than that in observation group. PAI in observation group was lower than that in control group within 1 week after fi lling, and VAS scores were also lower than those in control group at 1 d, 3 d, 5 d and 7 d after fi lling (P ＜0.05). At 3 months after fi lling, diff erence in total response rate between the two groups was not statistically signifi cant (P ＞0.05). Before fi lling, PES and WES were similar in the two groups (P＞0.05). At 3 months after fi lling, PES and WES in observation group were higher than those in control group, while PLI, PD and SBI were lower than those in control group (P＜0.05). Conclusion Compared with AH Plus+ warm vertical condensation, iRoot SP single apex root canal fi lling has higher fi lling effi ciency, milder pain after fi lling, higher success rate and more signifi cant aesthetic eff ect in patients with diabetes mellitus and chronic periapical periodontitis.
