摘要:
目的:探究强脉冲光联合重组牛碱性成纤维细胞生长因子(Recombinant Bovine Basic Fibroblast Growth Factor,rb-bFGF)对玫瑰痤疮患者炎症因子及皮肤镜特征变化的影响。方法:以笔者医院2023年1月-2024年1月诊治的120例玫瑰痤疮患者为研究对象,随机分为强脉冲光(Intense Pulsed Light,IPL)治疗组(IPL组)、rb-bFGF凝胶治疗组(rb-bFGF组)及联合组,每组40例。比较三组患者血清炎症因子[白细胞介素(Interleukin,IL)-6、IL-8、肿瘤坏死因子(Tumor Necrosis Factor,TNF)-α]水平、皮肤镜检测指标(红斑程度评分、毛细血管扩张评分、毛囊口改变评分及皮肤镜总评分)、研究者总体评估量表(Investigator’s Global Assessment,IGA)评分、临床疗效、不良反应发生情况。结果:三组患者随着治疗时间延长,血清IL-6、IL-8及TNF-α水平、红斑程度评分、毛细血管扩张评分、毛囊口改变评分及皮肤镜总评分均明显下降(P<0.05);治疗后不同时间点的联合组血清IL-6、IL-8及TNF-α水平、红斑程度评分、毛细血管扩张评分、毛囊口改变评分及皮肤镜总评分均明显低于IPL组与rb-bFGF组(P<0.05)。联合组的治疗总有效率为95.00%,明显高于IPL组(72.50%)及rb-bFGF组(65.00%)(P<0.05)。与治疗前相比,三组患者治疗3次后IGA评分均明显降低(P<0.05),且联合组显著低于IPL组和rb-bFGF组。三组间不良反应率差异无统计学意义(P>0.05)。结论:强脉冲光联合rb-bFGF凝胶可有效降低玫瑰痤疮患者炎症因子水平,显著改善皮肤镜特征,且安全性良好,为玫瑰痤疮的治疗提供了更优的联合方案。
Abstract:
Objective To explore the eff ects of intense pulsed light combined with recombinant bovine basic fi broblast growth factor (rb-bFGF) on infl ammatory factors and dermoscopic features of rosaceae patients. Methods A total of 120 patients with rosette acne admitted to our hospital from January 2023 to January 2024 were collected as the study objects, and were randomly divided into intense pulsed light treatment group (IPL group), rb-bFGF treatment group (rb-bFGF group) and combined group, with 40 cases in each group. The levels of serum infl ammatory factors [interleukin (IL) -6, IL-8, tumor necrosis factor (TNF) -α], dermoscopic detection indexes (erythema degree score, telangiectasia score, follicle opening change score and total dermoscopic score), Investigator’s Global Assessment (IGA) score, clinical effi cacy, and occurrence of adverse reactions were compared among the 3 groups. Results With the prolongation of treatment time, serum IL-6, IL-8 and TNF-α levels, erythema degree score, telangiectasia score, hair follicle opening change score and total dermoscopic score were signifi cantly decreased in 3 groups (P<0.05). The levels of IL-6, IL-8 and TNF-α in serum, erythema degree score, telangiectasia score, hair follicle opening change score and total dermoscopic score in combined group were signifi cantly lower than those in IPL and rb-bFGF groups at diff erent time points after treatment (P<0.05). The total eff ective rate of combination group was 95.00 %, which was signifi cantly higher than that of IPL group (72.50%) and rb-FGF group (65.00%) (P<0.05). Compared with before treatment, IGA score in 3 groups was signifi cantly decreased after 3 treatments (P<0.05), and the combined group was signifi cantly lower than that in IPL group and rb-bFGF group. There was no signifi cant diff erence in the adverse reaction rate among the three groups (P>0.05). Conclusion The combination of intense pulsed light and rb-bFGF can eff ectively reduce the level of infl ammatory factors in rosacea patients, signifi cantly improve the characteristics of dermoscopy, and provide a better combination scheme for the treatment of rosacea.
