目的：探讨下睑缩肌移位术单用及联合眼睑轮匝肌折叠缩短术治疗老年性下睑内翻的临床效果。方法：将2016年5月-2020年5月在笔者所在医院收治的老年性下睑内翻患者86例（116眼）作为研究对象，并将其分为观察组（43例，57眼）和对照组（43例，59眼），观察组给予下睑缩肌移位术+眼睑轮匝肌折叠缩短术联合治疗，对照组给予下睑缩肌移位术，评估两组术后近、远期临床疗效、眼睑美观的满意度评分，并记录两组术后并发症，比较术后1年的复发率。结果：术后1周、3个月，两组的短期疗效对比、眼睑美观的满意度评分比较，差异均无统计学意义（P＞0.05）；术后1年，观察组的远期疗效、满意度评分均高于对照组，复发率低于对照组，差异均有统计学意义（P＜0.05）；观察组并发症总发生率低于对照组（P＜0.05）。结论：下睑缩肌移位术联合眼睑轮匝肌折叠缩短术治疗老年性下睑内翻的远期疗效显著，能明显降低术后复发率，减少术后并发症，提高患者对术后眼睑美观的满意度。
Abstract:
Objective To explore the clinical effect of lower eyelid contractor muscle transfer alone and combined with orbicularis fold shortening in the treatment of senile entropion. Methods 86 cases (116 eyes) of elderly patients with lower eyelid entropion admitted to the author’s hospital from May 2016 to May 2020 were taken as the research objects, and they were divided into observation group (43 cases, 57 eyes) and control group (43 cases, 59 eyes), the observation group was given the combined treatment of lower blepharoconstrictor shift + orbicularis fold shortening, and the control group was given lower blepharoconstrictor shift. The two groups were evaluated at 1 week and 3 after surgery. The clinical curative effect and eyelid aesthetic satisfaction scores were recorded at the end of the month and 1 year after the operation, and the postoperative complications of the two groups were recorded, and the recurrence rate at 1 year after the operation was compared. Results At 1 week and 3 months after operation, there was no significant difference in the short-term curative effect and the satisfaction score of eyelid aesthetics between the two groups (P ＞0.05); 1 year after the operation, the long-term curative effect and satisfaction degree of the observation group The scores were higher than those of the control group, and the recurrence rate was lower than that of the control group. The difference was statistically significant (P＜0.05), the total incidence of complications in the observation group was lower than that of the control group (P＜0.05). Conclusion Lower eyelid retractor muscle transposition combined with fold eyelid orbicularis oculi muscle shortening has a significant long-term efficacy in the treatment of elderly lower eyelid inversion, and can significantly reduce postoperative recurrence rate, lower postoperative complications, and improve the satisfaction of postoperative eyelid aesthetics.
