摘要:
目的：评价Q开关Nd:YAG 1 064 nm激光联合美塑疗法在黄褐斑治疗中的临床疗效及安全性。方法：选取2019年5 月-2020年5月就诊于笔者皮肤科门诊的140例黄褐斑患者，随机分为两组，对照组（n=70）：采用Q开关Nd:YAG 1 064 nm激 光治疗，每次治疗间隔2周，共10次治疗；治疗组（n=70）：在对照组的基础上联合美塑疗法，每月1次。治疗后对比两组 MASI评分、临床疗效、皮肤生理指标检测、满意度及不良反应发生率，并随访3月观察复发率。结果：相较于对照组，治疗 组患者MASI评分显著降低（P＜0.05），有效率明显升高（65.71% vs 81.43%，P＜0.05）；与对照组相比，治疗组面颊部皮 损角质层含水量升高（P＜0.01）；经表皮水分流失、血色素、黑色素水平显著低于对照组（P＜0.01），患者满意度提高 （85.71% vs 95.7%），所有患者均未见严重不良反应。结论：Q开关Nd:YAG 1 064 nm激光联合美塑疗法治疗黄褐斑疗效优于 单一激光治疗，可明显改善患者临床症状，提升治疗有效率，增加患者满意度，值得在临床上推广应用。
Abstract:
Objective To evaluate the clinical efficacy and safety of Q-switched Nd:YAG 1064nm laser combined with methothrapy on melasma. Methods 140 cases of melasma patients were enrolled from May 2019 to May 2020.They were randomly divided into two groups: control group (n=70): Q-switched Nd:YAG 1 064 nm laser was used for 10 treatments, with an interval of 2 weeks each time. Treatment group (n=70): on the basis of control group, methothrapy was combined once a month. After treatment, the MASI scores, clinical efficacy, detection of skin physiological indexes,satisfaction and incidence of adverse reactions were compared between the two groups, and the recurrence rate was observed for 3 months. Results Compared with the control group, the MASI score of patients in the combined treatment group was significantly lower(P＜0.05), the effective rate was significantly increased(65.71% vs 81.45%, P＜0.05), and patient satisfaction was improved(85.71% vs 95.7%, P＜0.05). Compared with the control group, the water content of cuticle in buccal lesions in the treatment group was increased (P＜0.01), and the levels of transepidermal water loss, hemoglobin and melanin in the treatment group were significantly lower than those in the control group (P＜0.01),no serious adverse reactions were seen in all patients. Conclusion Q-switched Nd:YAG 1 064 nm laser combined with methothrapy for the treatment of melasma is better than that of single laser therapy. It can significantly improve clinical outcome, increase the satisfaction of patients. It is worthy of clinical application.
