摘要:
目的：观察十字形分牙法对复杂下颌水平低位阻生齿拔除患者的治疗效果。方法：选取2020年3月-2020年11月在笔者 医院口腔门诊就诊的下颌阻生齿患者进行术前CBCT检查，选取下颌水平低位阻生且伴有牙根数目≥2或牙根膨大的定为复杂 下颌水平低位阻生齿的患者，将纳入的患者随机分成对照组（n=30）和实验组（n=30）。对照组行常规微创拔除法，实验组 行十字形分牙拔除法，比较两组患者手术一般情况、面部肿胀、张口度、干槽症等并发症发生情况。结果：实验组患者手术 时间和术后出血持续时间分别为（42.18±3.11）min、（13.58±2.03）min，均短于对照组；术中VAS评分（3.73±0.68） 分，较对照组低，两组比较差异有统计学意义（P＜0.05）；实验组术后1 d和术后3 d的面部肿胀程度和张口受限程度较对照 组轻，差异有统计学意义（P＜0.05）；两组患者术后出血、干槽症等发生率无明显差别（F=0.197）。结论：十字形分牙法 对复杂下颌水平低位阻生齿拔除患者有较好的治疗效果，手术损伤小，具有良好的应用价值。
Abstract:
Objective To observe the therapeutic eff ect of the cruciform tooth division method on patients with low impacted teeth Extraction with complex mandibular level. Methods The patients with impacted mandibular teeth who were in the dental clinic of the authors’ hospital from March 2020 to November 2020 were selected for preoperative CBCT examination, and patients with mandibular impacted with low level of the mandible and accompanied by the number of roots ≥ 2 or root swelling was selected as a complex mandible patients with low-level impacted teeth were randomly divided into control group and experimental group. The control group underwent conventional minimally invasive extraction, and the experimental group underwent cross-shaped tooth extraction. The two groups were compared with general operation conditions, facial swelling, and mouth opening. The diff erence in the incidence of complications such as dry socket disease. Results The operation time and postoperative bleeding duration of the experimental group were (42.18±3.11) min and (13.58±2.03) min, respectively, which were shorter than those of the control group. The intraoperative VAS score was (3.73±0.68) points, which was lower than the control group. The diff erence was statistically signifi cant (P ＜0.05).The degree of facial swelling and the degree of mouth opening in the experimental group were less than those in the control group at 1 day and 3 days after the operation, and the diff erence was statistically signifi cant (P ＜0.05), There was no signifi cant diff erence in the incidence of postoperative bleeding and dry socket (F=0.197). Conclusion The cruciform tooth division method has a good therapeutic eff ect on patients with low impacted teeth with complex mandibular level, with little surgical damage, and has good application value.
