[摘要]目的:探讨CO2点阵激光对增生性瘢痕的临床疗效以及对TGF-β1表达的影响。方法:将144例增生性瘢痕患者随机分
为观察组(n =72)与对照组(n =72),观察组给予CO2点阵激光治疗,对照组给予皮损内曲安奈德注射治疗,治疗1个疗程(24
周)后对比两组患者临床有效率、瘢痕厚度及温哥华瘢痕量表(VSS)评分变化、瘢痕组织TGF-β1的表达情况。结果:观察
组总有效率为94.4%,显著高于对照组的79.2%(P <0.05);治疗前两组瘢痕厚度、瘢痕VSS评分、TGF-β1相对含量比较
差异均无统计学意义(P >0.05);治疗后两组患者瘢痕厚度、瘢痕VSS评分及TGF-β1相对含量均明显降低(P <0.05),
观察组明显低于对照组,差异具有统计学意义(P <0.05);两组患者常见不良反应发生率分别为12.5%、4.2%,差异无统
计学意义(P >0.05)。结论:CO2点阵激光治疗增生性瘢痕疗效确切,能够快速降低瘢痕厚度及瘢痕VSS评分,缓解临床症
状、体征,同时有效降低瘢痕组织中TGF-β1的表达。

Abstract:
Abstract: Objective To investigate the clinical effects and the influence on TGF-β1 of CO2 lattice laser in the treatment of
hypertrophic scar. Methods 144 patients with hypertrophic scar were randomly divided into the observation group(n =72) and
the control group(n =72), the observation group were treated with fractional CO2 laser, and the control group were treated with
intralesional injection of triamcinolone acetonide, the effective rate, scar thickness and VSS score, and TGF-β1 were contrasted
between the two groups after a course of treatment. Results The effective rate in the observation group was 94.4%, significantly
higher than 79.2% in the control group(P <0.05); there were no significantly difference in scar thickness, VSS score, and
TGF-β1 before treatment between two groups(P >0.05), but significantly reduced in the two groups after treatment(P <0.05),
and the observation group significantly lower than the control group after the treatment(P <0.05). The incidence of adverse
reactions was 12.5% and 4.2% in the two groups, there was no significantly difference(P >0.05). Conclusion Fractional
CO2laser has exactly clinical effects in the treatment of hypertrophic scar, it can quickly reduce the scar thickness, scar VSS
score, and relieve the clinical symptoms and signs, effectively reduce the expression of TGF-β1 in scar tissue.
