[摘要]目的：探讨上睑提肌腱膜瓣-额肌吻合术与上睑提肌缩短及徙前术对上睑下垂的手术效果差异。方法：选取笔者医院
2015年1月-2016年12月实施手术治疗的100例（180眼）重度上睑下垂患者进行回顾性分析，根据手术方法分为上睑提肌腱膜
瓣-额肌吻合术组（50例，94眼）、上睑提肌缩短及徙前术组（50例，86眼），比较两组患者的术后手术效果、术后不同时
间点的上睑回退量、并发症发生情况。结果：上睑提肌腱膜瓣-额肌吻合术组术后达到1级的有84眼（89.36%）、达到2级的
有10眼（10.64%）；上睑提肌缩短及徙前术组术后达到1级的有63眼（73.26%）、达到2级的有18眼（20.93%），两组比较
差异有统计学意义（P ＜0.05）。术后6个月、9个月、1年，上睑提肌腱膜瓣-额肌吻合术组的上睑回退量均显著低于同一时
间点的上睑提肌缩短及徙前术组，差异有统计学意义（P ＜0.05）。上睑提肌腱膜瓣-额肌吻合术组的术后并发症发生率为
4.26%显著低于上睑提肌缩短及徙前术组的15.12%，差异有统计学意义（P ＜0.05）。结论：上睑提肌腱膜瓣-额肌吻合术治
疗上睑下垂手术效果可靠、术后上睑回退量小、手术并发症少。

Abstract:
Abstract: Objective To investigate the effect of levator palpebral diaphragm-frontalis anastomosis on the upper eyelid and
the levator palpebral muscle shortening and anteversion on ptosis. Methods Selected 100 patients (180 eyes) with severe
blepharoptosis underwent surgical treatment in our hospital from January 2015 to December 2016 were retrospectively analyzed.
They were divided into two groups according to operative methods, the upper eyelid aponeurosis flap frontalis muscle anastomosis
group (50 cases, 94 eyes) and the levator palpebrae superioris shortening and preoperation group (50 cases, 86 eyes). The
postoperative outcomes, postoperative palpebral recession and operative complications of the two groups were compared. Results
There were 84 eyes (89.36%) who reached grade 1 and 10 eyes (10.64%) who reached grade 2 in the upper eyelid aponeurosis
flap frontalis muscle anastomosis group, 63 eyes (73.26%) reached grade 1 and 18 eyes (20.93%) reached grade 2 in the levator
palpebrae superioris shortening and preoperation group, the difference between the two groups was statistically significant (P＜0.05).
After 6 months, 9 months and 1 years, the upper eyelid receded in the upper eyelid aponeurosis flap frontalis muscle anastomosis
group was significantly lower than that in the levator palpebrae superioris shortening and preoperation group at the same time point
(P＜0.05). The incidence of postoperative complications in the upper eyelid aponeurosis flap frontalis muscle anastomosis group
was 4.26%, which was significantly lower than that in the levator palpebrae superioris shortening and preoperation group (15.12%,
P＜0.05). Conclusion The levator palpebral flap-fronto-abdominal anastomosis of the upper iliac crest is effective in treating the
sacroiliac crest, and the postoperative retrogradation of the palate is small and the complications are few.
