[摘要]目的:探讨上下颌扩弓联合矫正器矫治错牙合畸形患者的临床疗效。方法:选择2014年4月-2017年6月笔者科室行上下
颌扩弓联合直丝弓固定矫治器结束的32例青少年患者为研究对象,对患者矫正前后进行模型测量和头影测量分析。结果:
矫正后,下上颌牙弓宽度与牙弓周长均有一定增加,前磨牙区差值最大,其次为磨牙区,最后为尖牙区;上颌最大增加量
为(4.5±1.8)mm,差异有统计学意义(t =-5.992,P <0.05);下颌最大增加量为(6.1±1.7)mm,差异有统计学意义
(t =8.714,P <0.05);矫正前后上下颌牙弓长度均显著减小,差异有统计学意义(t =5.940,t =2.570,P <0.05);矫正
前后颌骨方面SNB、ANB与MP-SN等变化差异均有统计学意义(t =2.165,t =4.206,t =3.584,P <0.05);牙齿方面U1-SN、
U1-NA、覆盖等均显著减小,差异有统计学意义(t =18.791,t =9.166,t =5.940,t =5.522,P <0.05);矫正后L1-MP明显增
大,差异有统计学意义(t =4.502,P <0.05)。结论:上下颌扩弓联合矫正器治疗错牙合畸形可有效改善患者错牙合中牙弓不匹
配的情况,解除牙列拥挤,保持良好的咬合状态,并能改善患者颌面部美观。
Abstract:

Abstract: Objective To investigate the clinical effect of maxillary expansion combined with fixed orthodontics appliance in the
treatment of malocclusion. Methods 36 adolescent patients underwent the treatment with maxillary and mandibular expansion
combined with straight wire appliance in our hospital from April 2014 to June 2017 were selected. Pre-and post-treatment dental
casts and cephalometric radiographs were measured and compared. Results After treatment the maxillary expansion width and
arch perimeters were increased, the difference of the premolar area was the largest, followed by the molar region, and finally
the canine region; The maximum increment of maxillary was (4.5±1.8) mm, with statistic difference (t =-5.992, P <0.05); The
maximum increase of mandible was (6.1±1.7) mm, the difference was statistically significant (t =8.714, P <0.05). The length of
maxillary and mandibular arch decreased significantly after orthodontic treatment, and the difference was statistically significant
(t =5.940, t =2.570,P <0.05); The changes of SNB, ANB and MP-SN before and after orthodontic treatment were statistically
significant (t =2.165, t =4.206, t =3.584, P <0.05). The U1-SN, U1-NA, and coverage of teeth were significantly decreased, and
the difference was statistically significant (t =18.791, t =9.166,t =5.940,t =5.522,P <0.05); After correction, L1-MP increased
significantly (t =4.502, P <0.05). Conclusion Maxillary expansion combined with fixed orthodontics appliance in the treatment
of malocclusion can effectively improve the mismatch of the matching arch, relieve the crowding, maintain good occlusion, and
improve the facial appearance of the patients.
