[摘要]目的:通过锥形束CT(cone beam computer tomography,CBCT)评价不同垂直骨面型患者颌骨骨皮质厚度,为正畸临床
种植体支抗的使用提供参考依据。方法:选取(20~40岁)安徽医科大学附属口腔医院2012年-2017年拍摄过CBCT的129例正
畸患者,根据头影测量结果将其分为高角、均角、低角三组,每组43例。在每例患者的CBCT影像上选取4个位点(尖牙与第
一前磨牙之间、第一第二前磨牙之间、第二前磨牙与第一磨牙之间、第一二磨牙之间),分别测量上颌颊腭侧骨皮质以及
下颌颊侧骨皮质厚度进行比较。结果:在上颌,低角患者尖牙与第一前磨牙之间区域的颊侧骨皮质厚度为(1.64±0.45)
mm,显著高于均角患者的(1.39±0.24)mm和高角患者的(1.28±0.22)mm,差别具有统计学意义(P <0.05);在第一前
磨牙与第二前磨牙之间的区域,高角患者的骨皮质厚度(1.27±0.15)mm明显低于均角患者(1.45±0.22)mm和低角患者
(1.54±0.31)mm,差别具有统计学意义(P <0.05);第二前磨牙和第一磨牙之间,以及第一磨牙和第二磨牙之间的骨皮
质厚度具有类似的趋势。在下颌,牙槽骨颊侧骨皮质厚度与上颌牙槽骨结果趋势相同,均表现为高角患者骨皮质厚度最低,
其次是均角患者,骨皮质厚度最高的低角患者。结论:高角患者具有更低的皮质骨骨密度,针对这类患者,临床医生应该选
择一些额外增加支抗钉稳定性的措施。

Abstract:
Abstract:Objective To compare the cortical bone thickness of the maxilla and mandible alveolar process in different
vertical facial types using cone-beam computed tomography and provide reference for clinical orthodontists in using implant
anchorages. Methods 129 images of adult patients (20-40 years old) in the Affiliated Stomatologic Hospital of Anhui
Medical University from 2012-2017 were selected,and divided into 3 groups (low, normal, and high-angle) according to the
cephalometric analysis.The cortical bone thickness of four measurement sites (the interradicular areas between the canine
and first premolar,the first and second premolars, the second premolar and first molar,and the first and second molars) were
measured using cone-beam computed tomography. Results In the maxilla alveolar process, the buccal cortical bone thickness
of the interradicular areas between the canine and first premolar in the low angle patients was (1.64±0.45)mm, which was higher
than normal angle patients (1.39±0.24)mm and high angle patients (1.28±0.22)mm (P <0.05); the thickness of the interradicular
areas between the first and second premolars in the high angle patients was (1.27±0.15)mm,which was lower than normal angle
patients (1.45±0.22)mm and low angle patients (1.54±0.31)mm (P <0.05).The mandible alveolar process possessed the similar
trend with the maxilla one. Conclusion High angle patients had the lowest cortical bone thickness compared with normal and
low angle patients.Clinicians should apply some extra methods in enhancing the stability of implant anchorages for those high
angle patients.
