[摘要]目的:探讨微螺钉种植体支抗对骨性Ⅱ类高角患者露龈微笑的改善效果。方法:选择2016年1月-2018年1月在笔者医
院口腔正畸科收治的80例露龈微笑骨性Ⅱ类高角患者为研究对象,将微型螺钉支抗植入上颌两中切牙之间的牙槽骨中,两侧
下颌中切牙和下颌侧切牙之间设置牵引钩,微螺钉与牵引钩间行垂直牵引。观察患者的矫正效果,对露龈微笑矫治前(T0)、
前牙停止压入时(T1)、矫治结束后(T2)的X线头影的硬组织角度测量参数(SNA、SNB、ANB、MP-FH、U1-SN、L1-MP、U1-L1)、
硬组织线距测量参数(U6/PP、U1/PP、L1/MP、Overjet、Overbite)、软组织测量参数(U1-E1、L1-E1、Po-NB、GSL)进行对比
分析。结果:露龈微笑矫治的平均时间(5.12±3.44)个月,正畸治疗的平均时间(18.27±3.15)个月;患者的露龈微笑与面
型均有明显改善;T1时与T2时的SNA、ANB、MP-FH、U1-SN、U6/PP、U1/PP、L1/MP、Overjet、Overbite、U1-E1、L1-E1、
GSL明显小于T0时(P <0.05),U1-L1、Po-NB明显大于T0时(P <0.05);T2时的MP-FH、U6/PP、L1/MP、Overjet、Overbite、
U1-E1、L1-E1均明显小于T1时(P <0.05),U1-L1明显大于T1时(P <0.05)。结论:微螺钉种植体支抗对骨性Ⅱ类高角患者露
龈微笑具有明显的改善效果。

Abstract:
Abstracts: Objective To explore the improvement effect of micro-screw implant anchorage on skeletal classⅡ high angle
patients with gummy smile. Methods 80 cases of skeletal classⅡ high angle patients with gummy smile which was taken as
research object in the department of stomatology of our hospital during 2016.1-2018.1. The micro-screw implant anchorage
was implanted into the alveolar bone between the two middle incisors on the upper jaw, the traction hook was set on both
sides of the middle incisors of the lower jaw and between the lateral incisors of the lower jaw, and the micro - planting nail
was drawn vertically with the traction hook. The corrective effect was observed. The measurement parameters of X-ray head
sclerous tissues angle measurement parameter(SNA, SNB, ANB, MP-FH,U1-SN, L1-MP, U1-L1), sclerous tissues line distance
measurement parameters(U6/PP,U1/PP,L1/MP,Overjet,Overbite) and soft tissue measurement parameters(U1-E1,L1-E1,Po-
NB,GSL)was compared and analyzed before the correction of gummy smile (T0), when the anterior teeth stopped pressing (T1),
and after the treatment(T2). Results The average time of correction of gummy smile was (5.12±3.44) months and the average
time of orthodontic treatment was (18.27±3.15) months. The gummy smile and facial type of the patients were significantly
improved. At T1 and T2,the SNA,ANB,MP-FH,U1-SN,U6/PP,U1/PP,L1/MP,Overjet,Overbite,U1-E1,L1-E1 and GSL was
significantly smaller than that at T0(P <0.05), the U1-L1, Po-NB was significantly greater than that at T0(P <0.05). At T2,the
MP-FH, U6/PP,L1/MP, Overjet, Overbite, U1-E1,L1-E1 was significantly smaller than that at T1(P <0.05), the U1-L1 was
significantly greater than that at T1(P <0.05). Conclusion Micro-screw implant anchorage had obvious improvement effect on
skeletal classⅡ high angle patients with gummy smile.
