[摘要]目的:回顾性研究骨性Ⅲ类错牙合伴下颌偏斜患者进行正畸-正颌治疗各个治疗阶段的变化。方法:选取正畸-正颌联合
治疗的成人骨性Ⅲ类错牙合伴下颌偏斜畸形患者27例,通过对术前正畸、正颌手术及术后正畸阶段的X线头颅定位正位、侧位
片测量分析,研究正畸-正颌联合治疗不同治疗阶段颌骨、牙齿的变化。结果:术前正畸阶段,侧位片分析可见U1-NA距减小
2mm,L1-NB距增大1.87mm,U1-L1减少了11°,上下颌骨和正位片各项测量项目未见明显变化。正颌手术阶段,侧位片可见
SNA增大2.52°,SNB角减小4.85°,SND角减小3.95°,ANB角增大2.72°;正位片可见,下颌角间距平均增宽了4.65mm。非
偏斜侧髁突上点与下颌角点间距减小了2.13mm,偏斜侧髁突上点与下颌角点间距增大了2.51mm,非偏斜侧髁突上点与颏点间
距减小2.75mm,偏斜侧髁突上点与颏点间距增加6.13mm。术后正畸阶段,侧位片分析可见U1-NA角增大1.3°,且差异具有统
计学意义,正位片各项测量项目均未见统计学差异。结论:术前正畸阶段,牙齿角度发生明显变化,上前牙内收,下前牙唇
倾,但是术前去代偿往往不足;正颌手术阶段,颌骨位置明显改变,上颌前移,下颌后退,颏部偏斜也明显改善;术后正畸
过程中,牙齿与颌骨位置均有一定程度的复发趋势,但术后更接近正常值,术后面部偏斜也表现出复发趋势。

Abstract:
Abstract: Objective To study the changes in different clinical stages of orthodontic-orthognathic combined treatment in
patients with skeletal Class Ⅲ malocclusion and mandibular deviation. Methods 27 adult patients undergoing the orthodonticorthoganthic
combined treatment for mandibular protrusion and asymmetry are selected. Changes of teeth and mandible in
different stages are studied by analyzing the frontal and lateral X-rays captured before, during and after the surgery. Results
In the pre-operative orthodontic stage, the U1-NA distance decreases by 2mm, the L1-NB distance increases by 1.87mm, the
U1-L1 decreases by 11 degree, and the measured values of maxilla and mandible had no obvious changes. There is no obvious
change in the frontal X-ray. In the orthognathic operation stage, analysis of the lateral X-ray reveals that SNA increases by 2.52
degree, SNB decreases by 4.85 degree,SND decreases by 3.95 degree, and ANB increases by 2.72 degree. The frontal X-ray
shows that the distance between left and right gonion increases by 4.65mm. The distance between the condylion and gonion in
the deflected side decreases by 2.13mm, while the distance between the condylion and the gonion in the normal side increases
by 2.51mm.The distance between menton and the condylion in the deflected side decreases by 2.75mm, and the distance
between menton and the condylion in the normal side increases by 6.13mm. In the post-surgery orthodontic stage, lateral X-ray
analysis shows that the U1-NA angle significantly increases by 3.4 degrees. Conclusion During the pre-operative orthodontic
stage, the angle of teeth changes significantly.The upper incisors are retracted and the lower incisors are labial inclined. During
the orthognathic surgery, the position of the jaw significantly changes, the maxilla advances and the mandible retreats, and the
ankle deviation also significantly improves. In the post-operative orthodontic stage, the upper anterior teeth recur obviously, but
the occlusal relationship becomes better, the position of maxilla and the facial deviation shows the tendency of recurrence.
