摘要:

[摘要]目的：探讨清热利湿方联合1%吡美莫司乳膏治疗面部脂溢性皮炎的临床疗效。方法：选择2015年5月-2017年12月湖北
民族学院附属民大医院和恩施土家族苗族自治州中心医院共同收治的面部脂溢性皮炎患者146例，按照随机数字表法，分为
对照组（n =73）和观察组（n =73）。对照组外用1%吡美莫司乳膏进行治疗，观察组在对照组基础上联合使用清热利湿方进行
治疗。两组患者连续治疗4周。比较治疗后临床总有效率及治疗前后皮肤生理指标[皮肤含水量、经表皮失水量（TEWL）、皮
脂分泌率和pH值。分别于治疗前（T0）、治疗后1周（T1）、2周（T2）和4周（T3）检测患者血清组织蛋白酶S（Cat S）水
平，对比两组不良反应发生率和复发率。结果：观察组治疗后临床总有效率为84.93%，明显高于对照组的49.32%，差异有统
计学意义（P ＜0.05）；两组患者治疗后皮肤含水量明显高于治疗前，而TEWL和皮脂分泌率显著低于治疗前，同时观察组治
疗后各项皮肤生理参数改善情况均明显优于对照组，差异均有统计学意义（P ＜0.05）；与T0相比，两组患者T1、T2和T3时
血清Cat S水平均明显下降，且观察组治疗后各时间点血清Cat S水平均显著低于对照组，差异有统计学意义（P ＜0.05）；
随着治疗时间的延长，两组患者血清Cat S水平显著下降（P＜0.05）；两组患者不良反应发生率和复发率比较差异无统计学
意义（P ＞0.05）。结论：清热利湿方联合1%吡美莫司乳膏治疗面脂溢性皮炎疗效显著，能有效改善患者皮肤生理参数及血
清Cat S水平，安全性高、复发率低，值得临床推广应用。

Abstract:

Abstract: Objective To explore the clinical efficacy of recipe for Qingre Lishi combined with 1% pimecrilimus cream on
seborrheic dermatitis. Methods According to the random number table, 146 cases of patients admitted to the dermatology
department of our hospital from May 2015 to December 2017 were divided into control group (n =73) and study group (n =73).
Patients in the control group were treated with 1% pimecrilimus cream, while those in the study group were treated with recipe
for Qingre Lishi on the basis of the control group. Two groups of patients were treated for 4 weeks. The clinical total effective
rate of two groups of patients after treatment was compared. At the same time the skin physiology index[skin water content, the
epidermal water loss (TEWL), sebum secretion rate and pH value] of two groups before and after treatment were compared.
Moreover, the level of serum cathepsin S (Cat S) in patients was detected respectively before (T0), 1 week after the treatment
(T1), 2 weeks (T2) and 4 weeks (T3). In addition, the adverse reactions incidence and recurrence rate of two groups of patients
were compared. Results The total clinical efficiency of the study group was 84.93%, which was significantly higher than
the 49.32% of the control group, and the difference was statistically significant (P ＜0.05). After treatment, the skin moisture
content of the two groups was significantly higher than that of before treatment, while the TEWL and sebum secretion rate were
significantly lower than those of before treatment. Meanwhile, the improvement of various skin physiological parameters of the
study group was significantly better than that of the control group, and the differences were statistically significant (P ＜0.05).
Compared with T0, the serum Cat S levels of both groups at T1, T2 and T3 significantly decreased, while the serum Cat S levels
of the study group were significantly lower than those of the control group at all time points after treatment, and the differences
were statistically significant (P ＜0.05). Meanwhile, the serum Cat S level of the two groups significantly decreased with the
extension of treatment time (P ＜0.05). There was no statistically significant difference in the incidence and recurrence rate
of adverse reactions between the two groups (P ＞0.05). Conclusion The treatment of recipe for clearing heat and removing
dampness combined with 1% pimecrilimus cream on seborrheic dermatitis has remarkable curative effect, which can effectively
improve the skin physiological parameters and serum Cat S level of patients, and has low safety and recurrence rate, which is
worthy of clinical application.
