[摘要]目的:比较无托槽隐形矫治器与固定矫治器在正畸中对患者牙周健康的影响。方法:选取2017年1月-2018年11月在笔者
医院口腔正畸科就诊的68例牙周矫治患者,随机分为托槽组(34例)和无托槽组(34例)。托槽组为传统固定矫治器治疗,无
托槽组为无托槽隐形矫正技术治疗。分别观察并比较两组患者的转位牙矫正、倾斜牙矫正以及完成治疗所需的时间,牙周健康
指数,龈沟液中天门冬氨酸氨基转移酶(AST)和碱性磷酸酶(ALP)水平及并发症发生情况。结果:无托槽组患者的转位牙矫
正、倾斜牙矫正以及完成治疗所需的时间均低于托槽组(P<0.05)。治疗后,无托槽组患者龈沟出血指数(SBI)、茵斑指数
(PLI)水平均较治疗前升高(P<0.05),牙龈指数(GI)、龈沟探诊深度(SPD)水平的改变则无统计学差异(P >0.05)。
托槽组患者的GI、SBI、PLI水平均较治疗前升高(P<0.05),SPD的改变则无统计学差异(P>0.05),且无托槽组患者的GI、
SBI、PLI水平均低于托槽组(P<0.05),两组间的SPD则无统计学差异(P>0.05)。治疗后两组患者龈沟液中AST、ALP水平均
较治疗前升高,且无托槽组患者龈沟液中AST、ALP水平均低于托槽组(P<0.05)。无托槽组并发症的发生率为2.941%,托槽组
的并发症发生率为14.71%,两组对比具有统计学意义(P <0.05)。结论:在口腔正畸过程中,无托槽隐形矫治器较固定矫治器
对患者牙周健康的影响较小,可显著缩短治疗时间,减少正畸过程中并发症的产生,值得临床推广使用。

Abstract:
Abstract: Objective To compare the effects of non-bracket invisible appliance and fixed appliance on periodontal health in
orthodontic patients. Methods 68 patients with periodontal orthodontic treatment in the orthodontics department of our hospital
from January 2017 to November 2018 were randomly divided into the bracket group (34 cases) and the non-bracket group (34
cases). The bracket group was treated with traditional fixed appliance and the non-bracket group was treated with non-bracket
invisible orthodontic technique. The time needed to complete the treatment, periodontal health index, the levels of aspartate
aminotransferase (AST) and alkaline phosphatase (ALP) in gingival crevicular fluid and complications were observed and
compared between the two groups. Results The time required to correct the transposition teeth, oblique teeth and complete
the treatment in the non-bracket group were lower than those in the bracket group (P<0.05). After treatment, the levels of gingival
sulcus bleeding index (SBI) and plaque index (PLI) in the non-bracket group were higher than those before treatment (P<0.05),
while the levels of gingival index (GI) and the depth of gingival sulcus exploration (SPD) had no statistical difference (P>0.05). After
treatment, the levels of GI, SBI and PLI in the bracket group were higher than those before treatment (P<0.05), but the changes of the
level of SPD had no statistical difference (P>0.05), and the levels of GI, SBI and PLI in the non-bracket group were lower than those
in the bracket group (P<0.05), while there was no statistical difference in SPD between the two groups (P>0.05). After treatment,
the levels of AST and ALP in gingival crevicular fluid in the two groups were higher than those before treatment, and the levels
of AST and ALP in gingival crevicular fluid in the non-bracket group were lower than those in the bracket group (P <0.05). The
incidence of complications was 2.94% in the non-bracket group and 14.71% in the bracket group, with significant difference
between the two groups (P <0.05). Conclusion In the process of orthodontics, non-bracket invisible appliance has less effect
on periodontal health than fixed appliance. It can significantly shorten the treatment time and reduce the complications in the
process of orthodontics. It is worth popularizing and using in clinic.
