[摘要]目的:探讨氦氖激光治疗对大疱性类天疱疮皮损CXCR5、ICOS的影响及与疗效的关系。方法:选取2012年1月-2018年
8月于笔者医院诊治的84例大疱性类天疱疮患者为研究对象,采用随机数字法分为对照组和实验组,每组42例。两组均给予
糖皮质激素系统治疗,实验组在此基础上给予氦氖激光治疗,对比两组治疗前后泡液中CXCR5、ICOS表达水平及临床疗效。
结果:治疗前两组CXCR5、ICOS含量比较差异无统计学意义(P >0.05);与治疗前比较,治疗后2周、4周和6周疱液CXCR5、
ICOS含量明显降低,且实验组明显低于对照组,差异均有统计学意义(P <0.05)。与对照组比较,实验组无新疱长出时
间、皮损面基本无渗出时间和愈合时间均显著缩短,总有效率显著升高,差异均具有统计学意义(P <0.05)。治疗有效者
治疗前疱液中CXCR5和ICOS含量显著低于无效者,差异有统计学意义(P <0.05)。结论:糖皮质激素联合氦氖激光治疗大疱
性类天疱疮可有效提高疗效,这可能与疱液中CXCR5和ICOS表达水平相关。

Abstract:
Abstract: Objective To analyze the effect of helium-neon laser treatment on CXCR5 and ICOS in bullous pemphigoid lesions,
and to explore its relationship with curative effect. Methods Eighty-four patients with bullous pemphigoid who were treated in
our hospital from January 2012 to August 2018 were enrolled in the study. They were randomly divided into control group and
experimental group, 42 cases each in each group. Glucocorticoid system was given to both groups, and the experimental group
was given helium-neon laser treatment. The expression levels of CXCR5 and ICOS in blister fluid and clinical efficacy were
compared between the two groups. Results There was no significant difference in the content of CXCR5 and ICOS between
the two groups before treatment (P >0.05). Compared with before treatment, the content of CXCR5 and ICOS in blister fluid
decreased significantly at 2 weeks, 4 weeks and 6 weeks after treatment, and experimental group was significantly lower than
control group after treatment, the above differences were statistically significant (P <0.05). Compared with the control group,
no new blisters growth time, no oozing time and lesional healing time, and the total effective rate in the experimental group
were significantly increased (P <0.05). The content of CXCR5 and ICOS in blister fluid in effective group before treatment was
significantly lower than that in ineffective group (P <0.05). Conclusion The combined application of helium-neon laser can
effectively improve the curative effect of bullous pemphigoid, which might be related to the expression levels of CXCR5 and
ICOS in blister fluid.
