[摘要]目的：观察单侧完全性唇腭裂患儿唇裂硬腭一期修复对腭裂修复手术及患儿牙弓发育的影响。方法：40例年龄为3～6
月龄的单侧完全性唇腭裂患儿，随机分为两组。观察组：行唇裂及硬腭一期修复；对照组：仅行唇裂修复。然后两组患儿均
在1岁半时行腭裂修复。两次手术时分别取两组患儿上牙颌模型，测量上腭裂隙的宽度变化并进行比较；腭裂修复术时，对
比两组患儿出血量的多少以及所作松弛切口的差异性，对两组牙颌模型进行牙弓的长、宽测量并做比较。结果：通过对牙颌
模型的测量，发现在腭裂手术时观察组腭裂隙缩窄更明显，较对照组有显著性差异，具有统计学意义（P ＜0.05）；腭裂手
术时，观察组较对照组所作松弛切口更少，平均出血量也更少,均有显著性差异（P ＜0.05）；1岁半时，两组患儿牙弓发育
无明显差异（P ＞0.05）。结论：单侧完全性唇腭裂患儿唇裂及硬腭一期修复可使上腭裂隙缩窄更明显，腭裂修复时所作松
弛切口更少，减少了术后上腭骨面裸露面积，减轻了对患儿上颌骨生长的抑制作用,有利于患儿恢复；早期犁骨瓣关闭硬腭
短期内对牙弓发育无明显影响。

Abstract:
Abstract: Objective To observe the influence of coinstantaneous cleft lip and hard palate crack operation on palatoplasty
and dental arch growth in unilateral complete cleft lip and palate patient. Methods Fourty 4.0-6.0 months old patients with
unilateral complete cleft lip and palate were divided into equal number of two groups.The experimental group accepted onestage
repair of cleft lip and hard palate,the control group only received cleft lip operation. Dentognathic model of all patients
were acquired in cleft lip surgery and cleft palate surgery at the age of one and a half, the palate crack width of dentognathic
model were studied and comparison between two groups.The differences of the lateral relaxation incisions and blood loss were
compared between the two groups in palatoplasty. The length and width of dental arch were measured and compared in two
groups of dentognathic models during cleft palate repair. Results The dentognathic model were studied in all patients. The
data analysis showed the crack width became smaller in experimental group, the difference had obvious significance(v＜0.05).
The experimental group had fewer relaxation incisions and blood loss than the control group in palatoplasty,the difference
was obvious(P＜0.05). At the age of one and a half years, there was no significant difference in the dental arch growth between
the two groups(P＞0.05). Conclusion Simultaneous repairs of cleft lip and hard palate can make the palate crack width more
narrow in unilateral complete cleft lip and palate patient, the number of relaxation incision was less in palatoplasty.It reduces
postoperative denudation area of palate bone, the inhibitory effect of palatoplasty was lesser on the maxillary growth of
children. Less bleeding in the operation was beneficial to the recovery of children.The early vomer flap closure of hard palate
had no significant effect on dental arch development in the short term.
