[摘要]目的：探讨并评价重度先天性睑裂狭小综合征患者进行内外眦成形联合上睑提肌缩短术的矫治效果与手术技巧，为临
床治疗该类疾病提供支持。方法：回顾性分析总结1998年1月-2018年2月收治的重度先天性睑裂狭小综合征患者36例，其中
男22例，女14例，年龄5～28岁，平均年龄9.5岁。所有患者术前均存在内眦赘皮，鼻根部低平及内眦间距过宽，双侧重度上
睑下垂。先行内眦赘皮切除和内眦韧带预置缝线，并切断韧带打活结，暂不用缝合；并行外眦皮肤切开，用结膜组织修复外
眦角；再行上睑提肌缩短术矫正上睑下垂，依次行内眦韧带结扎并内眦成形。术后随访6～24个月。结果：所有患者均接受
一次性完成内外眦开大联合上睑提肌缩短术，术后患者内眦赘皮消失，泪阜显露，重睑线自然美观，无睑内翻、睑外翻和暴
露性角膜炎等并发症发生。结论：睑裂狭小综合征所致上睑下垂可以一期上睑提肌缩短术联合内外眦开大术进行矫正，这种
方法缩短了手术治疗周期，减少多次手术费用和痛苦，远期观察不易复发，手术效果确切可靠。

Abstract:
Abstract: Objective To investigate and evaluate the surgical skills and effect of the combined surgery of medial and lateral
canthoplasty and levator palpebrae muscle shortening on severe blepharophimosis-ptosis-epicanthusinversus syndrome(BPES),
and provide support for clinical treatment of this kind of disease. Methods The combined surgery was performed on 72 eyes of
36 cases of severe BPES during January 1998 and February 2018(22 males and 14 females, 5-28 years old, averaged 9.5). All
cases has epicanthus inversus, falted nasion, widened inner canthal distance and bilateral severe ptosis. During the operation, the
epicanthectomy was frisly performed and the inner canthus ligament was cut apart with pre-suture,following the outer canthus
dermotomy with outer-canthoplasty using conjunctiva, the Blephar-ophimosis-Ptosis was cured thereby. With the unrestricted
sursumversion, the palpebralis shorten operation was performed, thepre-suture oninner canthus ligament was ligatured and
the inner-canthoplasty was performed at last. The follow-up period varied form 6-24 months. Results Postoperatively, all the
medial epicanthus disappeared with exposed lacrimal caruncle. The double eyelid line after the combined surgery was natural
and beautiful, without complications like entropion or ectropion of eyelid, and exposure keratitis. Conclusion Ptosis caused
by BPES can be corrected by one stage shortening of palpebralis shorten combined with large canthotomy, which shortens
the surgical treatment cycle, reduces the cost and pain of multiple operations, and makes long-term observation less likely to
relapse.
